摘  要:目的:观察3D打印技术在A0分型B、C分型Pilon骨折治疗中的应用。方法:选取我院骨科2020年8月一2021年7月进行治疗的50例A0分型B、C分型Pilon骨折患者,以随机数字表法将患者分为对照组(n=25)、观察组(n=25),对照组使用常规螺旋CT进行扫描,观察组使用3D打印技术建立立体模型,对比2组手术质量、术前术后临床指标、术后恢复质量以及并发症发生率。结果:观察组切口长度、手术时间、术中出血量均低于对照组,P<0.05;观察组术中透视次数、骨折愈合时间以及住院时间均低于对照组,P<0.05;观察组随访时间与对照组对比无差异,P>0.05;手术前观察组HGB、HCT与对照组对比无差异,P>0.05,手术后观察组HCB、HCT均低于对照组,P<0.05;观察组AOFAS评分、Mazur评分高于对照组,VAS评分低于对照组,P<0.05;治疗前2组炎性指标对比无差异P>0.05,治疗后观察组TNF-α、IL-6、CRP均显著低于对照组,P<0.05;观察组术后并发症发生率低于对照组,P<0.05;治疗前2组CQ0L-74评分对比无差异,P>0.05,治疗后观察组生理功能、情感功能、物质生活和社会功能评分均高于对照组,P<0.05。结论:AO分型B、C分型Pilon骨折使用3D打印技术可以为患者制定个性化的手术方案,提升手术实施质量,提升术后恢复质量,控制并发症的发生率,对于骨折的恢复具有积极作用。Objective:To observe the application of 3D printing technology for AO classification B and C classification Pilon fracture treatment.Methods:50 patients with AO type B and C Pilon fractures,who volunteered to participate in the experiment,were admitted to the orthopedics department between August 2020 and August 2021 for treatment,the patients were divided into reference group n=25,test group n=25 by a random number table,the reference group was scanned using conventional spiral CT,and the test group was used 3D printing technology to establish stereological models,and the surgical quality,preoperative and postoperative clinical parameters were compared between the two groups quality of postoperative recovery as well as complication rates.Results:operative time,intraoperative blood loss,as well as hospital stay were lower in the test group than in the reference group at P<0.05.HGB and HCT in the test group before surgery did not differ from the reference group,P>0.05;HGB and HCT in the test group after surgery were lower than those in the reference group,P<0.05.AOFAS and Mazur scores were higher and VAS scores lower in the test group than in the reference group at P<0.05.The incidence of postoperative complications in the test group was lower than that in the reference group,P<0.05.Conclusion:AO subtyping B and C Pilon fractures using 3D printing technology can produce personalized surgical protocols for patients,enhance the quality of surgical implementation,enhance the quality of postoperative recovery,and control the incidence of complications,which has a positive effect on fracture recovery.
