共管模式干预老年髋部骨折患者功能恢复和生活质量的meta分析

摘　　要：目的:系统评价共管模式对老年髋部骨折患者预后和生活质量的影响。方法:计算机检索CNKI、WanFang、VIP、CBM、PubMed、EMbase、TheCochraneLibrary、WebOf Science数据库中共管模式影响老年髋部骨折患者预后及生活质量的随机对照研究,检索时限为从建库一2022年3月14日。由2名研究人员独立筛选文献,提取资料并评价纳入研究的偏倚风险,采用RevMan5.4软件进行meta分析。结果:最终纳入15篇文献,涉及2820个样本,观察组实行老年科与骨科协作的共管模式,对照组采用单一骨科病房治疗,Meta分析表明,观察组Harris评分[MD=8.90,95%CI(7.87,9.94),P<0.0001],BI量表[SMD=0.87,95%CI(0.02,1.71),P=0.04]、SF-36中生理健康领域评分[SMD=1.37,95%CI(0.31,2.43),P=0.01]2组差异有统计学意义,但精神健康领域[SMD=0.88,95%CI(-0.02,1.77),P=0.05]2组差异无统计学意义。结论:共管模式可以改善髋关节功能的恢复,提高其日常活动能力和生活质量。Objective:To systematically evaluate the effect of co-management mode on the prognosis and quality of life in elderly patients with hip fracture.Methods:A randomized control test ffecting prognosis and quality of life in elderly patients with CNKI,Wan-Fang,VIP,CBM,PubMed,EMbase,The Cochrane Library,Web Of Science database from database to March 14,2022.The literature was screened independently by two researchers,extracted data and evaluated the bias risk included in the study,with meta analysis using RevMan 5.4 software.Results:15 documents,Including 2,820 samples,The intervention group implements the co-management mode of geriatric department and orthopedic cooperation,The control group was treated with a single orthopedic unit,Meta analysis shows,In-tervention Group Harris score[MD=8.90,95%CI(7.87,.9.94),.P<0.0001],BI Scale[SMD=0.87,95%CI(0.02,.1.71),.P=0.04],Physiological Health field score in SF-36[SMD=1.37,95%CI(0.31,.2.43),.P=0.01]The two-group differences are statistically significant,But in the mental health areas[SMD=0.88,95%CI(-0.02,.1.77),.P=0.05]The two groups were not statistically significant.Conclusion:Management mode can improve hip function and daily ability and quality of life.
