摘 要:目的探讨全数字化乳腺X线摄影技术检查对乳腺BI-RADS 4类肿块的诊断效果。方法选取2021年1月—2022年12月在福建医科大学附属三明第一医院经全数字化乳腺X线摄影诊断为BI-RADS 4类乳腺肿块患者60例,所有患者均行病理检查,并与病理检查结果进行比较,分析全数字化乳腺X线摄影检查的诊断效果。结果全数字化乳腺X线摄影技术检查诊断BI-RADS 4A、4B、4C类乳腺肿块的敏感度分别为50.00%、88.89%、95.65%,特异度分别为93.33%、77.78%、50.00%,准确率分别为88.24%、83.33%、92.00%,Kappa值分别为0.4334、0.667、0.457。ROC曲线显示,全数字化乳腺X线摄影技术检查诊断BI-RADS 4类乳腺肿块的AUC值为0.873,标准误为0.040,P<0.001。结论全数字化乳腺X线摄影能够有效诊断BI-RADS 4类乳腺肿块,能够为临床乳腺肿块的良恶性诊断提供客观可靠的影像学依据,值得临床推广应用。Objective To explore the diagnostic effect of full-field digital mammography on BI-RADS class 4 breast masses.Methods Sixty patients with BI-RADS class 4 breast masses diagnosed by full-field digital mammography at Sanming First Hospital Affiliated to Fujian Medical University from January 2021 to December 2022 were selected.All patients underwent pathological examination.The results of full-field digital mammography were compared with the pathological examination to analyze the diagnostic effect of full-field digital mammography.Results The sensitivity of full-field digital mammography in the diagnosis of BI-RADS 4A,4B,and 4C breast masses were 50.00%,88.89%,and 95.65%,respectively.The specificity was 93.33%,77.78%,and 50.00%,respectively.The accuracy was 88.24%,83.33%,and 92.00%,respectively.The Kappa values were 0.4334,0.667,and 0.457,respectively.The ROC curve shows that the AUC value for diagnosing BI-RADS class 4 breast masses using full-field digital mammography is 0.873,with a standard error of 0.040(P<0.001).Conclusion Full-field digital mammography can effectively diagnose BI-RADS class 4 breast masses and provide objective and reliable imaging basis for the diagnosis of benign and malignant breast masses in clinical practice,which is worthy of clinical promotion and application.
