摘 要:目的探讨血清性别决定区Y框蛋白6(SOX6)、血小板P选择素62(CD62P)、网膜素1(Omentin-1)对急性心肌梗死(AMI)患者经皮冠脉介入术(PCI)后院内主要不良心脏事件(MACE)的预测效能。方法选取2018年8月至2021年8月收治的AMI患者126例作为研究对象。患者均行PCI治疗,统计PCI后院内MACE发生情况。根据术后院内是否发生MACE分为MACE组与无MACE组,比较两组临床资料和血清SOX6、CD62P、Omentin-1水平,分析各血清指标与临床指标的关系及对MACE的影响,并评价各血清指标对AMI患者PCI术后院内MACE的预测效能。结果126例AMI患者中,PCI术后院内MACE发生率为30.16%(38/126);MACE组左室射血分数(LVEF)、SOX6、Omentin-1低于无MACE组,心功能Killip分级、冠状动脉病变程度、血清CD62P水平高于无MACE组(P<0.05);血清SOX6、Omentin-1与LVEF呈正相关,CD62P与LVEF呈负相关(P<0.05);血清SOX6、Omentin-1与心功能Killip分级、冠状动脉病变程度呈负相关,CD62P与心功能Killip分级、冠状动脉病变程度呈正相关(P<0.05);血清SOX6、Omentin-1低水平患者PCI术后院内MACE发生风险是高水平患者的3.519、6.171倍,CD62P高水平患者PCI术后院内MACE发生风险是低水平患者的2.700倍;血清SOX6、CD62P、Omentin-1联合预测AMI患者PCI术后院内MACE的曲线下面积(AUC)为0.927,大于各血清指标单独预测(P<0.05)。结论血清SOX6、CD62P、Omentin-1表达与AMI患者心功能、病情程度及术后院内MACE发生有关;SOX6、Omentin-1降低,CD62P升高,可增大AMI患者PCI术后院内MACE发生风险,三者联合可提高对AMI患者PCI术后院内MACE的预测价值。Objective To investigate the predictive performance of serum sex-determining region Y-box protein 6(SOX6),platelet P-selectin 62(CD62P)and omentin-1(Omentin-1)on in-hospital major adverse cardiac events(MACE)in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods A total of 126 patients with AMI in our hospital from August 2018 to August 2021 were selected as the research subjects,all of whom underwent PCI,and the incidence of MACE in the hospital after PCI was counted.And they were divided into MACE group and nonMACE group based on whether MACE occurs in the hospital after PCI.The clinical data,serum levels of SOX6,CD62P and Omentin-1 were compared between the two groups,the relationship between serum indexes and clinical indexes and their influence on MACE were analyzed,and the predictive efficacy of each serum index on in-hospital MACE in patients with AMI after PCI was evaluated.Results Among the 126 AMI patients,the incidence of in-hospital MACE after PCI was 30.16%(38/126).The left ventricular ejection fraction(LVEF),SOX6,and Omentin-1 in the MACE group were lower than those in the non-MACE group.The degree of coronary artery disease and serum CD62P level were higher than those in the non-MACE group(P<0.05).Serum SOX6 and Omentin-1 were positively correlated with LVEF,and CD62P was negatively correlated with LVEF(P<0.05).Serum SOX6 and Omentin-1 were negatively correlated with cardiac function Killip grade and coronary artery disease degree,while CD62P was positively correlated with cardiac function Killip classification and coronary artery disease degree(P<0.05).The risk of in-hospital MACE in patients with low serum SOX6 and Omentin-1 levels underwent PCI was 3.519 and 6.171 times that of patients with high levels,and the risk of in-hospital MACE after PCI in patients with high levels of CD62P was 2.700 times that of patients with low levels.The AUC of MACE was 0.927,which was greater than that predicted by each serum index alone(P<0.05).Conclusion The expres
