摘 要:凝血功能障碍是已知的创伤性颅脑损伤(TBI)后遗症,可致TBI患者死亡率增加。血小板功能障碍已被确定为导致TBI后凝血功能障碍的因素之一,并与患者的不良预后相关。血小板功能障碍无论是源于直接损伤,还是由已有疾病或药物使用引起,准确监测血小板对治疗的反应是患者获得最优诊疗的关键。血小板输注被认为是治疗血小板功能障碍的一种潜在的干预措施,随着血小板功能检测技术向着精确化方向发展,以及学界对凝血过程中特定的凝血机制紊乱理解不断加深,使临床医生可基于血小板功能测试结果为患者制定个性化干预措施。由于目前对TBI后血小板功能障碍的病理生理学检测技术有限,因此对TBI后血小板功能障碍疗法的研发也进展缓慢。本文就TBI后血小板功能障碍的病理生理学、发生率、临床结局以及当前疗法等研究进展作一简要综述。Coagulopathy is a known sequelae of traumatic brain injury(TBI)and can increase the mortality of TBI patients.Platelet dysfunction has been identified as one of the factors leading to coagulation dysfunction after TBI and is associated with poor prognosis in patients.Whether platelet dysfunction is caused by direct injury,or by existing diseases or drug use,accurate monitoring of platelet response to treatment is the key to optimal diagnosis and treatment.Platelet infusion is considered as a potential intervention for the treatment of platelet dysfunction.With the development of platelet function detection technology toward accuracy,and the deepening academic understanding of specific coagulation mechanism disorders during coagulation,clinicians can develop personalized interventions for patients based on platelet function test results.Due to the limited pathophysiology and platelet function detection technology of post-TBI platelet dysfunction,the research and development of post-TBI platelet dysfunction therapy has also been slow to progress.This article reviews the pathophysiology,incidence,clinical outcomes and current therapies of platelet dysfunction after TBI.
