摘  要:目的比较锥形束CT(CBCT)与多层螺旋CT(MSCT)对致传导性聋的听骨链病变的显示能力。方法回顾性分析因听骨链病变致听力下降的118例传导性聋患者的颞骨CBCT或MSCT,记录听骨链病变类型。比较2种设备对听骨链病变的显示能力,并分别与术中所见对照,比较二者手术符合率差异。结果CBCT与MSCT对砧骨长脚部分或全部缺如和锤骨畸形的显示能力差异有统计学意义(χ^(2)=7.85,P=0.005和P=0.040),而2种设备在其他致传导性聋的听骨链病变显示能力差异无统计学意义(P>0.05)。CBCT显示听骨链病变的手术符合率高于MSCT(P=0.041)。结论CBCT在显示因砧骨长脚缺如和锤骨畸形导致的传导性聋方面优于MSCT,CBCT显示听骨链病变的手术符合率高于MSCT,CBCT对致传导性聋的部分听骨链病变的显示能力优于MSCT。Objective To compare the display ability of cone-beam computed tomography(CBCT)and multi-slice spiral computed tomography(MSCT)in ossicular chain lesions for conductive hearing loss.Methods CBCT or MSCT data from 118 patients with conductive hearing loss due to ossicular chain lesions was analyzed retrosqectively,and ossicular chain lesions were recorded.Then,the display ability of two devices in ossicular chain lesions was compared,and the coincidence rate of these lesions in two devices with operation was compared,respectively.Results There were significant statistical differences between two devices in the display ability of partial or total absence of the long limbs of incus and malleus deformity(χ^(2)=7.85,P=0.005 and P=0.040),whereas the display of other ossicular chain lesions for conductive hearing loss was no significant statistical differences(P>0.05).The coincidence rate of CBCT and operation in displaying ossicular chain lesions was higher than that of MSCT(P=0.041).Conclusion The display ability of CBCT is superior to MSCT in the display of absence of long limbs of incus and malleus deformity causing conductive hearing loss.CBCT has a higher surgical coincidence rate in displaying ossicular chain lesions than MSCT.Therefore,the display ability of CBCT in ossicular chain lesions for conductive hearing loss is better than that of MSCT.
