摘 要:目的探讨MRI信号强度增高椎间盘在强直性脊柱炎(AS)患者中的发病情况及椎间盘的MRI特点。方法回顾性分析106例AS患者共1617个椎间盘的MRI信号强度,并记录患者的年龄,分析椎间盘信号强度增高在颈、胸、腰椎的发病率;根据在T_(1)WI、T_(2)WI和脂肪抑制T_(2)WI的信号强度对这些椎间盘进行分型。结果16例(15.1%)AS患者共104个(6.4%)椎间盘显示信号强度增高,该组患者平均年龄高于其余AS患者(t=6.93,P<0.001)。信号强度增高椎间盘在颈椎的发病率高于胸椎(χ^(2)=13.1,P<0.001)和腰椎(χ^(2)=23.3,P<0.001)。共有32个1型椎间盘(在T_(1)WI、T_(2)WI和脂肪抑制T_(2)WI信号强度增高)、38个2型椎间盘(在T_(1)WI和T_(2)WI信号强度增高、脂肪抑制T_(2)WI信号强度减低)、16个3型椎间盘(在T_(2)WI和脂肪抑制T_(2)WI信号强度增高)。结论MRI信号强度增高椎间盘更常见于病程较长的AS患者的颈椎,是AS在脊柱的重要MRI征象之一。Objective To investigate the incidence of intervertebral discs with increased MRI signal intensity in patients with ankylosing spondylitis(AS),and the features of intervertebral discs on MRI.Methods The MRI signal intensity of 1617 intervertebral discs of 106 AS patients were analyzed retrospectively,the age of these patients were recorded.The incidence of intervertebral discs with increased signal intensity in cervical,thoracic and lumbar spine were analyzed.These intervertebral discs were classified according to the signal intensity on T_(1)WI,T_(2)WI and fat suppressed T_(2)WI.Results A total of 104(6.4%)intervertebral discs of 16 patients(15.1%)showed increased signal intensity,the mean age of this group patients was higher than that of the remaining AS patients(t=6.93,P<0.001).The incidence of intervertebral discs with increased signal intensity in cervical spine was higher than those in thoracic spine(χ^(2)=13.1,P<0.001)and lumbar spine(χ^(2)=23.3,P<0.001).A total of 32 type 1 intervertebral discs(with increased signal intensity on T_(1)WI,T_(2)WI and fat suppressed T_(2)WI),38 type 2 intervertebral discs(with increased signal intensity on T_(1)WI and T_(2)WI,decreased on fat suppressed T_(2)WI),16 type 3 intervertebral discs(with increased signal intensity on T_(2)WI and fat suppressed T_(2)WI)were observed.Conclusion The intervertebral discs with increased MRI signal intensity are more common in cervical spine of AS patients with long disease duration,which are one of the important MRI signs of AS in the spine.
