摘  要:目的基于系列运动伪影降低技术(SMART)结合多次信号平均,优化儿童腹部自由呼吸下三维T1高分辨率各向同性容积激发(3D e-THRIVE)序列图像质量。方法40例进行腹部3.0T MR检查的儿童纳入本研究,每例儿童均扫描5个结合SMART技术的横轴位3D e-THRIVE序列,5个序列的信号平均次数(NSA)分别为2、3、4、5、6次。2名资深放射医师采用4分法对5个序列的肝脏边缘清晰度、胰腺边缘清晰度、血管清晰度及总体图像质量进行主观评分,采用Kruskal-Wallis非参数检验方法分析主观图像质量评分。采用单因素方差分析法(ANOVA)比较5个序列的肝脏信噪比(SNR)、肝脏与门静脉间的对比噪声比(CNR),方差齐性者采用S-N-K进行组间两两比较,方差不齐者采用Dunnett T3进行组间两两比较。结果NSA为4、5、6次的3个3D e-THRIVE序列在主观图像质量评分及肝脏SNR上无明显统计学差异(P>0.05),优于NSA为2、3次的2个序列(P<0.05),NSA为5次的3D e-THRIVE序列中未出现完全不能满足诊断的病例;5个序列的肝脏与门静脉间的CNR无明显统计学差异。结论采用SMART技术结合4~6次NSA,可优化提升儿童自由呼吸下3D e-THRIVE序列的图像质量,为了避免完全不能诊断病例出现,建议选择SMART技术结合5次NSA。Objective To optimize image quality of abdominal free-breathing three-dimensional enhanced-T1 high resolution isotropic volume examination(3D e-THRIVE)sequence of children based on serial motion artifact reduction technique(SMART)with multiple signals averaging.Methods A total of 40 children who underwent abdominal MR examinations on a 3.0T scanner were included in this study.Each child was scanned with five transverse 3D e-THRIVE sequences combined with SMART,and the number of signals averaging(NSA)of the five sequences were 2,3,4,5 and 6 respectively.Two senior radiologists scored the image quality of the five sequences,including the hepatic edge sharpness,pancreatic edge sharpness,vascular clarity and overall image quality,using a 4-point scale,and the image quality score was analyzed by the Kruskal-Wallis non-parametric test.One-way analysis of variance(ANOVA)was used to compare the signal-to-noise ratio(SNR)of liver and contrast-to-noise ratio(CNR)between liver and portal vein,S-N-K used for pairwise comparison between groups with homogeneous variance,Dunnett T3 used for pairwise comparison between groups with uneven variance.Results There was no significant statistics difference in subjective image quality score and SNR of liver for the sequences with NSA of 4,5 and 6(P>0.05),which were significantly better than those of the sequences with NSA of 2,3(P<0.05),and there was no case completely failing to meet diagnostic requirement on the 3D e-THRIVE sequence with NSA of 5;there was no statistics difference in CNR between liver and portal vein among five sequences.Conclusion The usage of SMART combined with NSA of 4,5,6 can optimize the image quality of abdominal free-breathing 3D e-THRIVE sequence of children;in order to avoid the occurrence of cases that failed to meet the diagnostic requirements,SMART combined with NSA of 5 is recommended.
