五苓散抑制中性粒细胞胞外陷阱对糖尿病肾病肾小球内皮细胞的作用

摘　　要：糖尿病肾病是糖尿病最常见也是最严重的并发症之一。尿微量白蛋白是糖尿病肾病早期事件之一。肾小球内皮细胞是血液过滤中至关重要的元素,其受损与白蛋白尿的发生发展相关。高血糖状态下,细胞产生过多的中性粒细胞胞外陷阱(NETs),导致内皮细胞活性下降,凋亡增多,直接造成内皮功能障碍。NETs也可能通过放大炎症过程导致内皮损伤。五苓散经证明可以有效减少尿蛋白,保护内皮细胞,改善肾脏功能并延缓糖尿病肾病的进展。前期实验已证实五苓散能够减少炎症反应并降低NETs形成。五苓散可能通过抑制NETs形成,对肾小球内皮细胞产生保护作用,并抑制糖尿病肾病的发展,为中医药应用于糖尿病肾病的防治提供了一定的科学支持和实证依据。Diabetic nephropathy is one of the most common and severe complications of diabetes.Microalbuminuria in urine is one of the early events of diabetic nephropathy.The endothelial cells in the glomerulus are crucial elements in blood filtration,and their damage is correlated with the occurrence and development of albuminuria.In a state of high blood glucose,the cells produce an excessive amount of neutrophil extracellular traps(NETs),leading to reduced endothelial cell activity and increased apoptosis,resulting in endothelial dysfunction.NETs might also exacerbate the endothelial injury by amplifying the inflammatory response.Wuling San has been proven to effectively reduce urinary protein,protect endothelial cells,improve renal function,and delay the progression of diabetic nephropathy.Earlier experiments have demonstrated that Wuling San could decrease the inflammatory response and reduce NETs formation.In conclusion,Wuling San might exert a protective effect on glomerular endothelial cells by inhibiting the formation of NETs,thus suppressing the development of diabetic nephropathy.This offers a scientific basis and empirical evidence supporting the application of traditional Chinese medicine in the prevention and treatment of diabetic nephropathy.
