手掌横切口联合Bruner切口部分掌腱膜切除术治疗掌腱膜挛缩症的体会

摘　　要：目的 观察手掌横切口联合Bruner切口部分掌腱膜切除术治疗掌腱膜挛缩症的效果。方法 选取2012年7月至2020年12月河南科技大学第一附属医院收治的15例掌腱膜挛缩症患者为研究对象,均为单手,均采用手掌横切口联合Bruner切口部分掌腱膜切除术治疗。结果 15例患者中有14例切口均I期愈合,1例患者切口因皮下血肿致三角形皮瓣尖端皮肤部分坏死经换药后愈合。随访时间为11个月到5年,术后无1例复发,1例患者出现环小指一侧指体麻木、感觉减退,6个月后逐渐好转。按照中华医学会手外科学会上肢部分功能评定试用标准评定,优14例,良1例。结论 采用手掌横切口联合Bruner切口治疗掌腱膜挛缩症,临床效果满意,是治疗掌腱膜挛缩症的一种有效方法。Objective To observe the curative effect of transverse palm incision combined with Bruner incision on Dupuytren's contracture. Methods Fifteen patients with Dupuytren's contracture admitted to the First Affiliated Hospital of Henan University of Science and Technology from July 2012 to December 2020 were selected as research subjects, all of which were single handed. Partial palmar aponeurosis resection via palm transverse incision combined with Bruner incision was performed. Results There were 15 patients in this group, of which 14 patients had primary wound healing, and 1 patient had partial skin necrosis at the tip of triangular flap due to subcutaneous hematoma, which was healed after dressing change. The follow-up period was 11 months to 5 years. There was no recurrence after the operation. One patient had numbness and hypoesthesia of the finger body on the side of the ring finger and little finger, which gradually improved after 6 months. According to the trial criteria for evaluation of upper limb function of the Society of Hand Surgery of the Chinese Medical Association, 14 cases were excellent and 1 case was good. Conclusions The treatment of Dupuytren's contracture with transverse palmar incision combined with Bruner incision has satisfactory clinical effect and is an effective method for the treatment of Dupuytren's contracture.
