摘 要:目的探讨改良清洁消毒法对降低重症监护病房(ICU)住院患者多重耐药菌(MDRO)感染的效果。方法选取2021年急诊ICU收治的255例患者为对照组,2022年收治的248例患者为试验组;对照组采用常规清洁消毒方法,试验组在常规清洁消毒方法基础上采用改良清洁消毒法。观察2组患者MDRO感染率、三管感染发生率、菌落数达标率及患者病情转归情况。结果试验组患者MDRO感染率及三管感染率均低于对照组(P<0.05);改良式环境清洁消毒管理法实施后,空气、物体表面菌落数达标率均高于实施前(P<0.05),医务人员手部菌落数达标率差异无统计学意义;试验组患者治愈率高于对照组,未愈率及死亡率低于对照组(P<0.05),而好转率略低于对照组。结论改良清洁消毒法可有效降低ICU住院患者MDRO感染及导管相关性血流感染的发生率。Objective To understand the effect of improved environmental disinfection management on reducing the infection rate of multi-drug resistant organism(MDRO)in ICU patients.Methods A total of 255 patients in 2021 emergency ICU were selected as control group,and 248 patients in 2022 were selected as study group.The control group adopted the routine disinfection management measures,and the study group adopted the improved environmental cleaning and disinfection management method based on the routine disinfection management measures.The infection rate of multidrug-resistant bacteria,the incidence of three-tube infection,the standard rate of colony number and the prognosis of patients in the two groups were observed.Results The infection rates of MDRO and three tubes in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05);After the implementation of the improved environmental cleaning and disinfection management method,the standard rate of the number of colonies in air and object surface was higher than that before the implementation,and the difference was statistically significant(P<0.05).There was no significant difference in the rate of reaching the standard of the number of colonies in the hands of medical staff.The cure rate of study group was higher than that of control group,and the unhealed rate and death rate were lower than those of the control group.The differences were statistically significant(P<0.05).The disease improvement rate of the study group was slightly lower than that of the control group,and the difference was not statistically significant.Conclusion The modified environmental cleaning and disinfection method can effectively reduce the incidence of multidrug-resistant infections and catheter-related bloodstream infections in ICU inpatients.Effectively disinfecting the environment in ICU and improving the outcomes of patients in hospital are worthy of popularization and application.
