摘 要:骨质疏松已成为导致慢性肾衰竭血液透析患者残疾、死亡的主要原因。近年来,慢性肾衰竭血液透析患者骨质疏松相关病理机制的研究不断深入,认为其发病与钙磷代谢异常、维生素D缺乏、代谢性酸中毒、高血糖以及炎症反应相关,涉及Wnt/β联蛋白信号通路、骨形态发生蛋白相关信号通路、核因子κB受体活化因子(RANK)配体/RANK/破骨细胞抑制因子系统,且以上分子机制及信号通路均在慢性肾衰竭血液透析患者骨质疏松发生中起作用。Osteoporosis has become the main cause of disability and death in hemodialysis patients with chronic renal failure.In recent years,research on the pathological mechanisms related to osteoporosis in the hemodialysis chronic renal failure patients has been continuously deepened,and it is believed that its pathogenesis is related to abnormal calcium and phosphorus metabolism,vitamin D deficiency,metabolic acidosis,hyperglycemia,and inflammatory reactions,involving Wnt/β-catenin protein signaling pathway,bone morphogenetic protein related signaling pathway,receptor activator of nuclear factor-κB(RANK)ligand/RANK/osteoclast inhibitory factor system,and the above molecular mechanisms and signaling pathways all play a role in the occurrence of osteoporosis in the hemodialysis chronic renal failure patients.
