摘  要:目的探讨水脂分离多回波定量Dixon(Q-Dixon)技术与单体素高速多回波T2校正(HISTO)磁共振波谱技术对血液透析患者大腿肌肉脂肪含量测量结果的相关性及一致性。方法选择2022年2—9月在首都医科大学附属北京积水潭医院接受血液透析的72例患者为研究对象,使用3.0 T磁共振成像多回波Q-Dixon技术和HISTO技术对受试者大腿肌肉进行检查。多回波Q-Dixon技术所测得的脂肪分数(FF)图通过第三方软件ITK-SNAP分析,在图像中选取两个正方形感兴趣区,其大小等于HISTO体素的横截面,位置在HISTO体素内选取,将两感兴趣区内FF进行平均,将其与HISTO技术所得FF进行分析比较。结果股四头肌Dixon FF观察者内组内相关系数(ICC)为0.966(95%CI 0.949~0.979),Dixon FF观察者间ICC为0.958(95%CI 0.934~0.974)。多回波Q-Dixon测得股四头肌FF范围为2.0%~40.0%,中位6.1%(10.6%,4.3%)。HISTO测得股四头肌FF范围为1.0%~37.4%,中位5.8%(11.8%,4.3%)。多回波Q-Dixon技术与HISTO技术对于血液透析患者大腿股四头肌肌肉脂肪含量测量结果呈正相关(r_(s)=0.954,P<0.001);Bland-Altman图示多回波Q-Dixon技术与HISTO技术肌肉FF测量结果之间具有较高一致性差值均数为-0.19%;Wilcoxon符号秩和检验显示两种测量方法的FF比较差异无统计学意义(P>0.05)。结论多回波Q-Dixon技术与HISTO技术对于血液透析患者大腿肌肉脂肪含量测量结果具有明显的相关性和较好的一致性。Objective To investigate the correlation and consistency between the quantitative Dixon(Q-Dixon)technique using multiecho water-fat separation and the single voxel high-speed T2-corrected multiecho(HISTO)magnetic resonance spectroscopy technique in measuring the thigh muscle fat content of hemodialysis patients.Methods Seventy-two hemodialysis patients who received hemodialyis at Beijing Jishuitan Hospital,Capital Medical University from February to September 2022 were sincluded.The thigh muscles of the subjects were examined using the 3.0 T magnetic resonance multiecho Q-Dixon technique and the HISTO technique.The fat fraction(FF)images obtained using the Q-Dixon technique were analyzed using third party software ITK-SNAP.Two square regions of interest were selected in the images,with their size equivalent to the cross-sectional area of the HISTO voxel.The position was chosen within the HISTO voxel.The FF values within the two regions of interest were averaged and compared with the FF values obtained using the HISTO technique.Results The quadriceps Dixon FF intra-observer correlation coefficient(ICC)was 0.966(95%CI 0.949-0.979),and the Dixon FF inter-observer ICC was 0.958(95%CI 0.934-0.974).The quadriceps FF ranged from 2.0%-40.0%,median 6.1%(10.6%,4.3%).HISTO measured quadriceps FF ranged from 1.0%-37.4%,with a median of 5.8%(11.8%,4.3%).The multiecho Q-Dixon technique and the single voxel HISTO technique showed a positive correlation in measuring the quadriceps femoris muscle fat content of hemodialysis patients(r_(s)=0.954,P<0.001).The Bland-Altman plot indicated a high level of agreement between the Q-Dixon and single voxel HISTO techniques with an average difference of-0.19%in muscle FF measurements.The Wilcoxon signed-rank test revealed no statistically significant difference between the FF measurements of the two methods(P>0.05).Conclusion The multiecho Q-Dixon technique and the single voxel HISTO technique exhibit a significant correlation and good consistency in measuring the thigh muscle fat content o
