摘 要:目的研究激光+血管内皮生长因子(VEGF)抑制剂治疗糖尿病视网膜病变的临床效果。方法80例糖尿病视网膜病变患者,采用随机数字表法分成常规组和研究组,各40例。常规组采用激光治疗,研究组采用激光+VEGF抑制剂治疗。比较两组患者治疗效果及治疗前后VEGF水平、黄斑中心凹视网膜厚度(CMT)、矫正视力、眼内炎症因子。结果研究组患者治疗总有效率97.50%明显高于常规组的62.50%,差异具有统计学意义(P<0.05)。治疗后,研究组患者VEGF水平(121.36±1.21)ng/L、黄斑中心凹视网膜厚度(216.33±2.01)μm均低于常规组的(165.33±2.01)ng/L、(242.36±2.01)μm,矫正视力(4.53±0.33)高于常规组的(4.16±0.33),差异有统计学意义(P<0.05)。治疗后,研究组患者眼内肿瘤坏死因子-α(21.51±0.33)ng/L、白细胞介素-6(14.51±0.32)ng/L均低于常规组的(33.53±1.01)、(22.56±0.33)ng/L,差异有统计学意义(P<0.05)。结论激光与VEGF抑制剂联合治疗糖尿病视网膜病变疗效显著,且可帮助患者改善视力、黄斑中心凹视网膜厚度及眼内炎性因子指标,值得临床推广。Objective To study the clinical effect of laser photocoagulation combined with vascular endothelial growth factor(VEGF)inhibitor on diabetes retinopathy.Methods A total of 80 patients with diabetes retinopathy were divided into the conventional group and the study group according to the random numerical table,with 40 cases in each group.The conventional group was treated with laser photocoagulation,and the study group was treated with laser photocoagulation and VEGF inhibitor.The therapeutic effect,VEGF levels,central macular thickness(CMT),corrected visual acuity and intraocular inflammatory factors before and after treatment were compared between the two groups.Results The total effective rate of 97.50%in the study group was significantly higher than that of 62.50%in the conventional group,and the difference was statistically significant(P<0.05).After treatment,VEGF of(121.36±1.21)ng/L and central macular thickness of(216.33±2.01)μm in the study group were lower than those of(165.33±2.01)ng/L and(242.36±2.01)μm in the conventional group;the corrected visual acuity of(4.53±0.33)in the study group was higher than that of(4.16±0.33)in the conventional group;the differences were statistically significant(P<0.05).After treatment,the intraocular tumor necrosis factor-αof(21.51±0.33)ng/L and interleukin-6 of(14.51±0.32)ng/L in the study group were lower than those of(33.53±1.01)and(22.56±0.33)ng/L in the conventional group,and the difference were statistically significant(P<0.05).Conclusion The combination of laser photocoagulation and VEGF inhibitor is effective in the treatment of diabetes retinopathy,and can help patients improve their visual acuity,central macular thickness and inflammatory factors in the eye,which is worthy of clinical promotion.
