某儿童专科医院PIVAS细胞毒药物调配情况分析及精准调配实践总结

摘　　要：目的分析某儿童专科医院静脉用药集中调配中心(PIVAS)细胞毒药物调配情况,以降低细胞毒药物调配的差错率,保障患者用药安全、有效。方法对某儿童专科医院PIVAS细胞毒药物的调配差错数据进行归纳、分析,并采取相应的防范措施。结果2021年10月~2022年10月间共收集细胞毒药物调配差错133件。归纳后,细胞毒药物在调配全程中的差错主要分为三类:医嘱差错、排药差错和调配差错。医嘱差错类型中,溶媒选择不适宜占比较最大(51.85%),其次为溶媒量选择不适宜(33.33%),给药途径不适宜(4.63%),排名医嘱差错的前三项均为超说明书用药。排药差错类型中,排药数量差错比较最大(54.55%),其次为排药差错(27.27%),输液规格差错(18.18%)。调配差错类型中,未选用药品说明书指定溶解溶媒最大(57.14%),其次为调配输液未盖章(21.43%),漏抽溶媒(14.29%),漏加、多加药物最低(7.14%)。结论对细胞毒药物实施精准调配,是保障患者治疗安全与有效的重要环节,是保证药物疗效以及客观评估治疗方案的重要前提。Objective To analyze the situation of cytotoxic drug dispensing in pharmacy intravenous admixture services(PIVAS)of a paediatric specialist hospital,so as to reduce the error rate of cytotoxic drug dispensing,and ensure the safety and effective medication administration for patients.Methods The data on dispensing errors of PIVAS cytotoxic drugs in a children's hospital were summarized and analyzed,and corresponding preventive measures were taken.Results From October 2021 to October 2022,a total of 133 dispensing errors of cytotoxic drugs were collected.After summary,the errors in the whole process of cytotoxic drugs can be divided into three categories:medical order errors,medication dispensing errors,and dispensing errors.Among the types of medical order errors,inappropriate choice of solvent accounted for the largest proportion(51.85%),followed by inappropriate choice of solvent quantity(33.33%)and inappropriate route of administration(4.63%).The top three errors in medical order were off-label drug use.Among the types of medication dispensing errors,dispensing quantity error was the largest(54.55%),followed by dispensing sequence error(27.27%)and infusion specification error(18.18%).Among the types of dispensing errors,failure to select the solvent specified in the drug instructions was the highest(57.14%),followed by failure to seal the dispensing infusion(21.43%),the missing solvent extraction(14.29%),and missed or excessive addition of drugs(7.14%).Conclusion Precision dispensing of cytotoxic drugs is an important part of ensuring the safety and effectiveness of patient treatment,and is an important prerequisite for ensuring drug efficacy and objective assessment of treatment protocols.
