单侧下肢肿胀的病因分析并文献复习

摘　　要：目的探讨单侧下肢肿胀的病因,旨在为鉴别诊断提供依据。方法收集本院169例非深静脉血栓性单侧下肢肿胀患者的临床病历资料,并检索Pubmed、中国知网和万方3个数据库进行文献复习。对最终获得的临床病历资料进行分析、整理,分析单侧下肢肿胀的病因。结果共有573例(本院169例、文献检索404例)单侧下肢肿胀患者的临床资料纳入最终研究,其病因可总结归纳为6大类,其中下肢出血及血肿形成89例(15.53%),淋巴回流障碍42例(7.33%),软组织感染39例(6.81%),血管本身病变157例(27.40%),周围组织压迫242例(42.23%),其他4例(0.70%)。结论引起单侧下肢肿胀的病因较多,需通过详细询问病史、仔细体格检查,结合相关辅助检查、检验,多角度分析,以明确病因、减少误诊。Objective To evaluate the etiology of unilateral lower extremity edema for providing basis for differential diagnosis.Methods The clinical data of 169 patients with unilateral lower extremity edema admitted to our hospital was collected.Pubmed,CNKI and Wanfang databases were searched for literature review.All the clinical medical data was analyzed and sorted out to clarify the etiology of unilateral lower extremity edema.Results A total of 573 patients(169 cases in our hospital and 404 cases in literature search)with unilateral lower extremity edema were included in the final study.The etiology was summarized and classified into 6 categories,including 89 cases(15.53%)of lower extremity hemorrhage and hematoma,42 cases(7.33%)of lymphatic lymphatic obstruction,39 cases(6.81%)of soft tissue infection,157 cases(27.40%)of vascular disease,and 242 cases(42.23%)of surrounding tissue compression and other 4 cases(0.70%).Conclusion There are many etiologies of unilateral lower extremity edema.So it is necessary to make a detailed medical history inquiry,careful physical examination,combined with relevant auxiliary examination,and analyze from multiple perspectives to clarify the etiology and reduce misdiagnosis.
