益骨汤联合经皮穿刺椎体成形术治疗胸腰椎骨质疏松性骨折的效果及对患者血清Omentin-1、TGF-β1、SF表达的影响

摘　　要：目的探讨益骨汤联合经皮穿刺椎体成形术(PVP)治疗胸腰椎骨质疏松性骨折的疗效及对血清网膜素-1(Omentin-1)、转化生长因子-β1(TGF-β1)、铁蛋白(SF)的影响。方法选取该院2021年1月至2023年6月间收治的胸腰椎骨质疏松性骨折患者120例,采用随机数字法分为对照组与观察组,各60例。对照组行PVP治疗,术后补充钙剂;观察组在对照组基础上另给予补肾益骨汤治疗。比较两组治疗前后下腰疼痛视觉模拟评分法(VAS)评分,血清Omentin-1、TGF-β1及SF水平,Oswestry功能障碍指数(ODI)、骨密度(BMD)及Cobb角变化。结果两组术后VAS评分均呈现下降趋势,组内术后1、3、6个月时不同时间VAS评分比较差异有统计学意义(P<0.05),观察组术后1、3、6个月VAS评分均低于对照组(P<0.05);术前,两组Omentin-1、TGF-β1、SF、ODI、BMD及Cobb角比较差异无统计学意义(P>0.05);术后6个月,两组Omentin-1、TGF-β1及BMD均升高,SF、ODI及Cobb角均降低(P<0.05),观察组术后6个月时Omentin-1、TGF-β1及BMD高于对照组,ODI及Cobb角低于对照组(P<0.05),两组术后6个月时SF及总有效率比较(对照组88.33%,观察组93.33%)比较差异无统计学意义(P>0.05)。结论益骨汤结合PVP可有效缓解胸腰椎骨质疏松性骨折患者疼痛程度,提高Omentin-1、TGF-β1水平,降低SF水平,促进骨折愈合,改善脊椎功能,临床疗效满意。Objective To investigate the efficacy of Yigu Decoction combined with percutaneous vertebroplasty(PVP)in the treatment of thoracolumbar osteoporotic fractures and the effects on Omentin-1,transforming growth factor-β1(TGF-β1)and ferritin(SF).Methods A total of 120 patients with thoracolumbar osteoporotic fracture admitted to the hospital from January 2021 to June 2023 were selected and divided into control group and observation group by random number method,with 60 cases in each group.The control group was treated with PVP and supplemented with calcium after operation.Observation group was also given bushen Yigu decoction on the basis of control group.VAS scores,serum concentrations of Omentin-1,TGF-β1 and SF,Oswestry disability index(ODI),bone mineral density(BMD)and Cobb Angle were compared between the two groups before and after treatment.Results The VAS scores of the two groups showed a downward trend and the VAS scores of the observation group at 1,3 and 6 months were significantly different at different times(P<0.05).The VAS scores of the observation group at 1m,3m and 6m were lower than those of the control group(P<0.05).Before surgery,there were no significant differences in Omentin-1,TGF-β1,SF,ODI,BMD and Cobb angles between the two groups(P>0.05).6 months after operation,Omentin-1,TGF-β1 and BMD in both groups were increased and SF,ODI and Cobb angles were decreased(P<0.05).Omentin-1,TGF-β1 and BMD in observation group were higher than those in control group,and ODI and Cobb angles were lower than those in control group(P<0.05).There was no significant difference in SF and total effective rate(88.33%in control group and 93.33%in observation group)between the two groups at 6 months after surgery(P>0.05).Conclusion Yigu Decoction combined with PVP can effectively relieve the pain degree of patients with thoracolumbar osteoporotic fractures,increase the levels of Omentin-1 and TGF-β1,reduce the level of SF,promote fracture healing,improve spinal function,and the clinical efficacy is satisfactory.
