摘 要:目的:探讨磁共振成像(MRI)、电子计算机断层扫描(CT)、X线联合诊断早期强直性脊柱炎(AS)骶髂关节病变的价值。方法:选取2020年6月—2022年6月平原县第一人民医院收治的256例强直性脊柱炎患者作为研究对象,患者均行MRI、CT、X线检查,以术后组织病理检查结果作为“金标准”,比较不同检测方式诊断AS骶髂关节病变的效能。结果:256例AS患者经术后病理检查显示156例患者确诊为AS骶髂关节病变,不同检测方式诊断AS骶髂关节病变的准确度、灵敏度比较,MRI与3项联合检测>CT>X线,MRI与3项联合检测诊断AS骶髂关节病变的特异度高于CT,不同检测方式对关节间隙变窄、关节面侵蚀的检出率比较,MRI与3项联合检测>CT>X线,不同检测方式对关节软骨肿胀、关节面骨质囊变的检出率比较,3项联合检测>MRI>CT>X线,不同检测方式对Ⅰ、Ⅱ级检出率比较,MRI与3项联合检测>CT>X线,差异有统计学意义(P<0.05)。结论:相较于X线与CT,MRI诊断早期AS骶髂关节病变的价值更显著,且3项联合检查可提升早期AS骶髂关节病变特征的检出率。Objective:To explore the value of magnetic resonance imaging(MRI),computed tomography(CT),and X-ray com⁃bined examination in the diagnosis of sacroiliac joint lesions in early ankylosing spondylitis(AS).Methods:A total of 256 patients with ankylosing spondylitis admitted to the First People′s Hospital of Pingyuan County from June 2020 to June 2022 were selected as the study subjects.All patients underwent MRI,CT and X-ray examinations.The results of postoperative histopathological ex⁃amination were used as the"gold standard"to compare the efficiency of different detection methods in diagnosing AS sacroiliac joint lesions.Results:Postoperative pathological examination showed that,among 256 patients with AS,156 cases were diagnosed with AS sacroiliac joint lesions.The accuracy and sensitivity of different detection methods in the diagnosis of AS sacroiliac joint lesions were compared and ranked as follows:MRI and the combined detection>CT>X-ray.The specificity of MRI and the com⁃bined detection in the diagnosis of AS sacroiliac joint lesions was higher than that of CT.The detection rate of joint space narrow⁃ing and articular surface erosion by different detection methods was compared and ranked as follows:MRI and the combined detec⁃tion>CT>X-ray.The detection rate of articular cartilage swelling and articular surface bone cyst degeneration by different detec⁃tion methods was compared and ranked as follows:the combined detection>MRI>CT>X-ray.The detection rate of Grade I and II by different detection methods was compared and ranked as follows:MRI and the combined detection>CT>X-ray.The differ⁃ence was statistically significant(P<0.05).Conclusion:Compared with X-ray and CT,MRI has a more significant value in diagnos⁃ing early AS sacroiliac joint lesions,and the combined detection of three examinations can increase the detection rate of early AS sacroiliac joint lesions.
