摘　　要：目的:探讨冠心病合并幽门螺杆菌(Hp)感染患者血清中基质金属蛋白酶-2(MMP-2)、血管内皮素-1(ET-1)、白细胞介素-17(IL-17)的表达水平。方法:选取2017年1月—2022年5月昌邑市人民医院收治的冠心病患者80例作为研究对象,均进行13C-尿素呼气试验,按照Hp感染情况分为Hp阳性组(n=42)与Hp阴性组(n=38)。收集患者资料,检测MMP-2、ET-1、IL-17水平,分析3种因子水平的影响因素,并分析MMP-2水平与ET-1、IL-17水平的关系。结果:Hp阴性组血清MMP-2、ET-1、IL-17水平低于Hp阳性组,差异有统计学意义(P<0.001)。不同Hp感染时间、冠心病确诊时间患者MMP-2、ET-1、IL-17水平比较,差异无统计学意义(P>0.05);多支血管病变患者MMP-2、ET-1、IL-17水平高于单只血管病变患者,差异有统计学意义(P<0.05)。MMP-2水平与ET-1、IL-17水平呈正相关(r=0.287,0.283,P=0.013,0.014)。结论:冠心病合并Hp感染患者血清中MMP-2、ET-1、IL-17水平升高,MMP-2水平与ET-1、IL-17的表达呈正相关,且3种因子高表达易导致患者出现多支血管病变。Objective:To explore the expression levels of matrix metalloproteinase-2(MMP-2),vascular endothelin-1(ET-1),interleukin-17(IL-17)in serum of patients with coronary heart disease and Helicobacter pylori(Hp)infection.Methods:A total of 80 patients with coronary heart disease treated in Changyi People's Hospital from January 2017 to May 2022 were selected as the study subjects.All patients received 13C-urea breath test.They were divided into Hp positive group(n=42)and Hp negative group(n=38)according to the Hp infection.The data of patients were collected.The levels of MMP-2,ET-1 and IL-17 were detected.The influencing factors of the levels of three factors were analyzed.The relationship between MMP-2 levels and ET-1 and IL-17 levels was analyzed.Results:The levels of serum MMP-2,ET-1 and IL-17 in the Hp negative group were lower than those in the Hp positive group,and the difference was statistically significant(P<0.001).There was no significant difference in the levels of MMP-2,ET-1 and IL-17 in patients with different Hp infection time and diagnosis time of coronary heart disease(P>0.05);the levels of MMP-2,ET-1 and IL-17 in patients with multiple vascular lesions were higher than those in patients with single vascular lesion,and the difference was statistically significant(P<0.05).The levels of MMP-2 were positively correlated with the levels of ET-1 and IL-17(r=0.287,0.283,P=0.013,0.014).Conclusion:The levels of MMP-2,ET-1,and IL-17 in the serum of coronary heart disease patients with Hp infection increase,the levels of MMP-2 are positively correlated with the levels of ET-1 and IL-17,and high expression of three factors may lead to multiple vascular lesions.
