摘　　要：目的:探讨成人外周血有核红细胞(NRBC)联合红细胞计数在贫血诊断中的应用价值。方法:选取2018年4月—2022年3月玉溪市中医医院收治的贫血患者268例作为贫血组,另选取同期阵发性睡眠性血红蛋白尿症、血小板减少性紫癜患者279例作为对照组,分别检测两组外周血中NRBC与红细胞计数水平,绘制受试者工作特征(ROC)曲线评估NRBC联合红细胞计数对贫血的诊断效能,比较不同贫血类型患者的有核红细胞水平。结果:贫血组外周血NRBC阳性率、NRBC绝对计数高于对照组,红细胞计数低于对照组,差异有统计学意义(P<0.001)。NRBC与红细胞计数联合诊断贫血的AUC为0.854[95%CI(0.821~0.888)],灵敏度为92.5%,特异度为73.1%。不同类型贫血患者外周血NRBC绝对计数与NRBC百分比由高到低依次为溶血性贫血、失血性贫血、巨幼细胞性贫血、缺铁性贫血、再生障碍性贫血,差异有统计学意义(P<0.05)。结论:外周血NRBC联合红细胞计数可用于诊断贫血,在一定程度上可鉴别贫血类型。Objective:To explore the application value of combined peripheral blood nucleated red blood cells(NRBC)and red blood cell count in the diagnosis of anemia in adults.Methods:A total of 268 anemia patients admitted to Yuxi Municipal Hospital of Traditional Chinese Medicine from April 2018 to March 2022 were selected as the anemia group,and 279 patients with paroxysmal nocturnal hemoglobinuria and thrombocytopenic purpura were selected as the control group.The levels of peripheral blood nucleated red blood cells and red blood cell count were detected in two groups.Receiver Operating Characteristic(ROC)curve was plotted to evaluate the diagnostic efficiency of NRBC combined with red blood cell count for anemia,and the levels of nucleated red blood cells in patients with different types of anemia were compared.Results:The positive rate of peripheral blood NRBC and absolute NRBC count in the anemia group were higher than those in the control group,and the red blood cell count in the anemia group was lower than that in the control group,and the difference was statistically significant(P<0.001).The AUC of NRBC combined with red blood cell count in the diagnosis of anemia was 0.854[95%CI(0.821-0.888)],the sensitivity was 92.5%,and the specificity was 73.1%.The peripheral blood NRBC level and NRBC percentage in patients with different types of anemia were hemolytic anemia,hemorrhagic anemia,megaloblastic anemia,iron deficiency anemia,and aplastic anemia in the descending order,and the difference was statistically significant(P<0.05).Conclusion:The peripheral blood NRBC combined with red blood cell count can be used to diagnose anemia and distinguish the type of anemia.
