基于患者视角的PICC健康教育在食管癌化疗患者护理中的应用效果

摘　　要：目的:探讨基于患者视角的经外周静脉穿刺中心静脉置管(PICC)健康教育在食管癌化疗患者护理中的应用效果。方法:选取2020年5月—2022年5月黄冈市罗田县人民医院收治的行PICC的食管癌患者60例作为研究对象,采用随机数字表法分为对照组和观察组,各30例。对照组接受常规健康教育,观察组接受基于患者视角的PICC健康教育。比较两组护理效果。结果:护理后,两组汉密尔顿抑郁量表、汉密尔顿焦虑量表评分低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05);两组自我护理能力、自我概念、自我责任感、健康知识水平评分及总分高于护理前,且观察组高于对照组,差异有统计学意义(P<0.05);两组睡眠紊乱、睡眠潜伏期、睡眠持续性、习惯性睡眠效率、主观睡眠质量、日间功能紊乱、催眠药物应用评分及总分低于护理前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P=0.037)。结论:基于患者视角的PICC健康教育在食管癌化疗患者护理中的应用效果显著,可改善其心理状态,提高自我护理能力、睡眠质量,降低并发症发生率。Objective:To explore the application effect of health education of peripherally inserted central catheter(PICC)based on the patient's perspective in the nursing of patients with esophageal cancer undergoing chemotherapy.Methods:A total of 60 patients with esophageal cancer undergoing PICC who were admitted to the People's Hospital of Luotian County in Huanggang City from May 2020 to May 2022 were selected as the study subjects.They were divided into the control group and the observation group according to random number table method,with 30 cases in the each group.The control group received routine health education,and the observation group received PICC health education based on the patient's perspective.The nursing effects of the two groups were compared.Results:After nursing,the scores of Hamilton Depression Rating Scale and Hamilton Anxiety Rating Scale in the two groups were lower than those before nursing,the scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The self-care ability,self-concept,self-responsibility,health literacy score and total score in the two groups were higher than those before nursing,the scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The sleep disorder,sleep latency,sleep persistence,habitual sleep efficiency,subjective sleep quality,daytime dysfunction,hypnotic drug use score and total score in the two groups were lower than those before nursing,the scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.037).Conclusion:PICC health education based on patients'perspective has significant application effects in patients with esophageal cancer undergoing chemotherapy,which can improve their psycho
