摘　　要：目的:探究联合使用重组人脑利钠肽(rhBNP)与沙库巴曲缬沙坦对急性心力衰竭(AHF)患者心功能及神经内分泌激素水平的影响。方法:选取兴山县人民医院2020~2021年收治的100例AHF患者为研究对象,按照简单随机化法将研究对象1∶1分为观察组50例和对照组50例,两组患者均常规使用扩血管药物进行降压治疗,对照组在此用药基础上加用沙库巴曲缬沙坦,观察组在对照组基础上加用rhBNP,比较两组治疗前、后心功能指标、神经内分泌细胞因子及炎性因子水平,两组用药后不良反应发生率的差异。结果:治疗后,两组左心室舒张末期容积(LVEDV),左心室收缩末期容积(LVESV)、血浆醛固酮(ALD)、心房钠尿肽(ANP)、心肌肌钙蛋白(cTnI)、白细胞介素-6及C反应蛋白水平均比治疗前下降,左心室射血分数(LVEF)比治疗前升高,且观察组LVEDV,LVESV、ALD、ANP、cTnI、白细胞介素-6及C反应蛋白水平比对照组更低,LVEF比对照组更高,差异均有统计学意义(P<0.05)。结论:rhBNP联合沙库巴曲缬沙坦对AHF患者的心功能有明显改善作用,可有效缓解心肌损伤,并能调节其神经内分泌系统,从而缓解炎性反应,药物方案安全性良好。Objective To investigate the effects of combined use of recombinant human brain natriuretic peptide(rhBNP)and sar-cubatrixartan on cardiac function and neuroendocrine hormone levels in patients with acute heart failure(AHF).Method 100 AHF patients admitted to our hospital from January 2020 to December 2021 were selected as the study subjects.According to a simple ran-domization method,the study subjects were divided 1∶1 into an observation group of 50 cases and a control group of 50 cases.Both groups of patients were treated with vasodilators for blood pressure reduction,while the control group was treated with sacubatravalsartan on top of this medication.The observation group was treated with rhBNP on top of the control group,and the cardiac function indicators before and after treatment were compared between the two groups,The levels of neuroendocrine cytokines and inflammatory factors,as well as the differences in the incidence of adverse reactions between the two groups after medication.Results After treatment,the lev-els of LVEDV,LVESV,ALD,ANP,cTnI,interleukin-6,and C-reactive protein decreased in both groups,while LVEF increased(P<0.05).Moreover,the levels of LVEDV,LVESV,ALD,ANP,cTnI,interleukin-6,and C-reactive protein in the observation group were lower than those in the control group,and LVEF was higher than those in the control group(P<0.05).Conclusion The combi-nation of rhBNP and sakubactria valsartan has a significant improvement effect on cardiac function in patients with AHF.It can effec-tively alleviate myocardial injury and regulate the neuroendocrine system,thereby alleviating inflammatory reactions.The drug regimen has good safety and can be widely applied in clinical practice.
