摘  要:目的:分析血清中促代谢因子(Betatrophin)、白介素1β(IL-1β)、血管内皮生长因子(VEGF)与糖尿病视网膜病变(DR)的相关性。方法:选取2021年3月至2022年6月住院的2型糖尿病(T2DM)患者,将患者分为3组,未合并视网膜病变(NDR)组,非增殖期(NPDR)组及增殖期(PDR)组,另选取同期住院无糖尿病患者(NC)37例作为对照组,检测患者血清Betatrophin、IL-1β、VEGF水平,采用组间比较分析Betatrophin、IL-1β、VEGF与糖尿病视网膜病变相关性,采用Logistics回归及调整模型方法探讨影响糖尿病视网膜病变的影响因素,通过绘制受试者工作曲线(ROC)评估3种因子对于DR及DR分期的诊断价值。结果:与NDR组相比,NPDR组及PDR组患者血清中Betatrophin、IL-1β及VEGF均升高,且三组间差异均有统计学意义(P<0.05);在T2DM患者中Betatrophin、IL-1β及VEGF具有正相关关系(P<0.05);Logistic回归分析表明,Betatrophin、IL-1β及VEGF为糖尿病视网膜病变危险因素。血清Betatrophin预测DR的ROC曲线下面积0.89,IL-1β的ROC曲线下面积0.71,VEGF的ROC曲线下面积0.84,预测PDR患者血清中IL-1β的ROC曲线下面积为0.83,Betatrophin的ROC曲线下面积为0.76,VEGF的ROC曲线下面积为0.72。结论:Betatrophin、IL-1β及VEGF参与DR的发生发展,Betatrophin对于DR及严重程度具有预测作用。Objective:To analyze the correlation between serum Betatrophin,interleukin-1β(IL-1β),vascular endothelial growth factor(VEGF)and diabetic retinopathy(DR).Methods:Patients with type 2 diabetes mellitus(T2DM)who were hospitalized from March 2021 to June 2022 were selected and divided into three groups:non-diabetic retinopathy(NDR)group,non-proliferative diabetic retinopathy(NPDR)group and proliferative diabetic retinopathy(PDR)group.In addition,37 patients without diabetes mellitus(NC)were selected as the control group.The levels of serum Betatrophin,IL-1βand VEGF were detected.The correlation between Betatrophin,IL-1β,VEGF and diabetic retinopathy was analyzed by comparison between groups.Logistic regression and adjustment model methods were used to explore the influencing factors of diabetic retinopathy,and the diagnostic value of the three factors for DR and DR staging was evaluated by drawing the receiver operating curve(ROC).Results:Compared with the NDR group,the serum Betatrophin,IL-1βand VEGF in the NPDR group and the PDR group were increased,and the differences among the three groups were statistically significant(P<0.05).Betatrophin,IL-1βand VEGF were positively correlated in T2DM patients(P<0.05).Logistic regression analysis showed that Betatrophin,IL-1βand VEGF were risk factors for diabetic retinopathy.When predicting DR patients,the area under the ROC curve of serum Betatrophin was 0.89,the area under the ROC curve of IL-1βwas 0.71,and the area under the ROC curve of VEGF was 0.84;When predicting PDR patients,the area under the ROC curve of serum IL-1βwas 0.83,the area under the ROC curve of Betatrophin was 0.76,and the area under the ROC curve of VEGF was 0.72.Conclusion:Betatrophin,IL-1βand VEGF are involved in the development of DR,and Betatrophin has a predictive role in DR and its severity.
