卵巢子宫内膜样癌的超声表现及临床特征

摘　　要：目的分析卵巢子宫内膜样癌(OEC)超声表现及临床特征,为临床早期诊断及治疗提供一定的理论依据。方法选取由病理证实45例OEC患者临床资料及超声图像。结果45例OEC患者平均年龄48.0岁,多以腹部包块、阴道出血、腹痛腹胀就诊。其中37例CA125水平升高,29例CA19-9水平升高。经病理证实21例合并子宫内膜癌,23例合并子宫内膜异位症(51%)。声像图表现:39例单侧,6例双侧,25个单房囊肿有实性成分;17个多房囊肿有实性成分;3个多房囊肿无实性成分;6个实性。实性部分及隔上彩色血流评分多为2~3分,阻力指数(RI)平均0.46,腹水少见。结论OEC声像图有一定特征,但是缺乏特异度,结合临床表现、患者年龄、CA125及CA19-9水平可以提高OEC术前诊断率。Objective To analyze the ultrasonic findings and clinical features of ovarian endometrioid carcinoma(OEC),and to provide theoretical basis for early diagnosis and treatment.Methods The clinical data and ultrasound images of 45 patients with OEC diagnosed and treated in the selected.Results The average age of the 45 patients with OEC was 48.O years old and most patients complained of abdominal mass,vaginal bleeding,abdominal pain and distension.Among them,37 cases had elevated CA125 level and 29 had elevated CA19-9 level.21 cases were pathologically confirmed to be complicated with endometrioid carcinoma.Endometriosis was found in 23 cases(51%).Ultrasonographic findings:There were 39 unilateral cysts,6 bilateral cysts,while 25 unilocular cysts had solid components.17 cases of multilocular cysts had solid components.3 cases of multilocular cysts had no solid component;and 6 cases demonstrated solid component.The solid part and septal color blood flow scores were mostly 2-3 points,the average resistance index(RI)was 0.46,and ascites was rare.Conclusions OEC sonogram has certain characteristics,but lacks specificity.Combining clinical manifestations,patient age,CA125 and CA19-9 levels can improve the preoperative diagnosis rate of OEC.
