摘 要:目的探讨超声在儿童骶尾部包块中的诊断思路。方法对经手术及病理证实的113例儿童骶尾部包块病例按照年龄分组,回顾性分析其超声声像图特征与临床资料。结果113例患儿分为大龄组(≥4岁)和小龄组(<4岁):大龄组29例中良性27例,恶性2例;小龄组84例中49例良性,30例恶性,还包含5例未成熟畸胎瘤。大龄组良性病变超声表现中囊性、囊实性及实性分别为12例、8例及7例;2例恶性均为实性。小龄组良性病变超声表现中囊性、囊实性及实性分别为19例、25例及5例;30例恶性病变表现囊实性及实性分别为10例和20例。两组儿童骶尾部良恶性包块的超声表现差异有统计学意义(P<0.05)。结论超声表现为囊性和囊实性的儿童骶尾部包块以良性多见,大龄儿童恶性包块比较少见,而小龄儿童恶性占比有所提升。因此,超声检查可以作为儿童骶尾部包块定位定性诊断的重要辅助工具。Objective To explore the diagnostic approach of ultrasound in pediatric sacrococcygeal masses.Methods We retrospectively analyzed 113 cases of pediatric sacrococcygeal masses with surgical and pathological confirmation,categorized by age.We reviewed the ultrasound features and clinical data.Results Among the l13 cases,there were two age groups:older(≥4 years)and younger(<4 years).In the older group(29 cases),27 were benign,and 2 were malignant.In the younger group(84 cases),49 were benign,30 were malignant,and 5 were immature teratomas.Among the older group's benign lesions,12 were cystic,8 were cystic-solid,and 7 were solid.Both malignant cases in this group were solid.Among the younger group's benign lesions,19 were cystic,25 were cysticsolid,and 5 were solid.In the malignant cases of this group,10 were cystic-solid,and 20 were solid.There was a statistically significant difference in the ultrasound characteristics of benign and malignant sacrococcygeal masses in both age groups(P<0.05).Conclusions Pediatric sacrococcygeal masses with cystic and cystic-solid ultrasound appearances are more likely to be benign.Malignant masses are less common in older children but more prevalent in younger children.Therefore,ultrasound examination is an important adjunctive tool for localizing and characterizing sacrococcygeal masses in children.
