从“聚于胃,关于肺”理论探讨小儿咳嗽的治疗

摘　　要：基于《素问·咳论篇》中“聚于胃,关于肺”理论,从肺、脾胃的功能联系、病机及治疗探讨小儿咳嗽的诊疗思路。肺脾胃生理功能联系紧密,在经络上相连相通;脾胃为气血生化之源,肺朝百脉,三脏气血相生相通,彼此润泽;脾主升清,胃主降浊,肺气以降为顺,三脏升降有序,润燥相济。小儿脾胃不足,若脾胃失调,一则气血精微难以上充于肺,肺卫不实,易感外邪;二则水液失调,易生痰湿,小儿体禀纯阳,痰易热化,从经络上传于肺,肺失宣降,发为咳嗽。在治疗小儿咳嗽时当遵循“聚于胃,关于肺”之意,以健运脾胃、宣畅肺气为原则,以断生痰之源、运贮痰之器,审证求因,灵活加减,以期取得较好的临床疗效。Based on the theory of“accumulating in the stomach,obstruction in the lungs”in Suwen·Cough Theory,this paper discusses the diagnosis and treatment of children’s cough from the functional relationship between the lung,spleen and stomach,pathogenesis and treatment.The physiological functions of lung,spleen and stomach are closely related,and the three viscera are connected on the meridians.The spleen and stomach are the source of qi and blood generation and circulation.The function of the lungs occurs through various meridians and acupoints in the human body,and the function of the lungs is manifested through each meridian and acupoint.The function of various meridians and acupoints in the human body are ultimately reflected in the lungs.The three organs of the spleen,stomach,and lungs are interconnected and moistened with qi and blood.The spleen is responsible for ascending clear,the stomach is responsible for descending turbid,the lung qi is descending in order,the three or⁃gans are ascending and descending in an orderly manner,and moistening and drying are coordinated.Children often have insufficient spleen and stomach,so they are prone to spleen and stomach disorders.Once the spleen and stom⁃ach disorders occur,firstly,it is difficult for the qi,blood,and essence to fill the lungs,and the lung health will not be solid,making it susceptible to external pathogens.Secondly,the imbalance of water and fluid can easily lead to phlegm and dampness.Children have a pure yang body,and phlegm can easily heat up.It spreads from the meridians to the lungs,causing the lungs to lose circulation and fall,resulting in coughing.When treating pediatric cough,the principle of“accumulating in the stomach,obstruction in the lungs”should be followed,with the principles of nour⁃ishing the spleen and stomach,promoting the circulation of lung qi,cutting off the source of phlegm production and storing phlegm,examining the syndrome to find the cause,and flexibly using classical prescription,in order to achieve better clin
