摘 要:心室预激性心肌病临床上起病隐匿, 多数无明显临床症状, 往往在症状出现时已有心脏扩大、心功能不全等。与心室预激相关的旁道是引起心室预激性心肌病的重要病因, 及时阻断旁道可有效逆转部分甚至全部心功能。药物治疗、导管消融治疗均被应用于心室预激性心肌病的治疗。随着三维标测技术的发展以及治疗手段的成熟, 儿童心室预激性心肌病的手术成功率高、并发症低, 且在低龄、低体重患儿方面不断取得进展。冷冻消融是传统射频消融的有益补充, 为心室预激性心肌病提供了一种安全有效的消融方法。该文就儿童心室预激性心肌病研究进展作一综述。While most patients with pre-excitation-induced cardiomyopathy have insidious onset and no obvious clinical symptoms in the early stage,cardiac enlargement and insufficiency often manifest when symptoms appear.The accessory pathway is the primary etiology of pre-excitation-induced cardiomyopathy,and its blockade could result in partial or complete resolution of the left ventricular ejection fraction.Both drug therapy and catheter ablation have been used to treat pre-excitation-induced cardiomyopathy.And the outcome of catheter ablation in pediatric patients has demonstrated a higher rate of success,less incidence of complications and especially notable advancements in the treatment of young children with low body mass due to the development of three-dimensional electro-anatomic mapping technology and the progress in therapy.Moreover,cryoablation is a significant supplement to traditional radiofrequency ablation,which has provided a relatively safe and effective treatment method for pre-excitation-induced cardiomyopathy.This paper reviews the research progress of pre-excitation-induced cardiomyopathy in children.
