摘  要:目的探讨早期自然流产(ESA)绒毛组织中血红素氧合酶-1(HO-1)、单核细胞趋化蛋白-1(MCP-1)蛋白表达及意义。方法选取2017年1月至2019年1月安庆市第一人民医院收治的149例ESA患者作为研究组,另选取同期145例行人工流产的正常妊娠孕妇作为对照组,采用免疫组化法检查两组绒毛组织HO-1、MCP-1蛋白表达情况。研究组患者均接受3年跟踪随访,根据患者是否进展为复发性流产(RA)将其分为RA组和非RA组,比较RA组和非RA组患者一般资料,采用多因素Logistic回归分析法分析RA的危险因素。结果研究组绒毛组织HO-1蛋白阳性表达率及阳性表达水平均低于对照组(P<0.05),研究组绒毛组织MCP-1蛋白阳性表达率及阳性表达水平均高于对照组(P<0.05);研究组149例患者在随访3年间有4例失访,余145例患者有22例进展为RA,RA发生率为15.17%(22/145);RA组绒毛组织HO-1蛋白阳性表达水平低于非RA组(P<0.05),MCP-1蛋白阳性表达水平高于非RA组(P<0.05);多因素结果显示,绒毛组织HO-1蛋白阳性表达低水平、MCP-1蛋白阳性表达高水平均为研究组患者RA的危险因素(P<0.05)。结论ESA患者相较于正常妊娠患者绒毛组织HO-1蛋白阳性表达率及阳性表达水平降低,MCP-1蛋白阳性表达率及阳性表达水平升高,绒毛组织HO-1蛋白阳性表达低水平、MCP-1蛋白阳性表达高水平均为ESA患者RA的危险因素。Objective To investigate the expression and significance of heme oxygenase-1(HO-1)and monocyte chemotactic protein-1(MCP-1)protein in villous tissue of early spontaneous abortion(ESA).Methods 149 patients with ESA admitted to Anqing First People's Hospital from January 2017 to January 2019 were selected as the study group,and 145 nor-mal pregnant women with induced abortion were selected as the control group in the same period.The expression of HO-1 and MCP-1 protein in villus were detected by immunohistochemistry.The patients in the study group were followed up for 3 years,and they were divided into RA group and non-RA group according to whether they had progressed to recurrent abortion(RA).The general data of patients in RA group and non-RA group were compared,and the risk factors of RA were analyzed by mul-tivariate Logistic regression analysis.Results The positive expression rate and level of HO-1 protein in villus tissue of the study group were lower than those of the control group(P<0.05),and the positive expression rate and level of MCP-1 protein in villus tissue of the study group were higher than those of the control group(P<0.05).During the 3 year follow-up period,4 of the 149 patients in the study group were lost,and 22 of the remaining 145 patients developed RA,with an incidence of 15.17%(22/145).The positive expression level of HO-1 protein in RA group was lower than that in non-RA group(P<0.05),and the positive expression level of MCP-1 protein in RA group was higher than that in non-RA group(P<0.05).Multivariate results showed that low level of HO-1 protein positive expression and high level of MCP-1 protein positive expression in vil-lus tissue were risk factors for RA in patients in the study group(P<0.05).Conclusion Compared with normal pregnancy,the positive expression rate and level of HO-1 protein in vllus tissue of ESA patients are lower,and the positive expression rate and level of MCP-1 protein are higher.The low level of HO-1 protein positive expression and the high level of MCP-1 protein
