摘 要:目的 观察电针预处理心经经穴对心肌缺血再灌注损伤(myocardial ischemia-reperfusion injury, MIRI)小鼠下丘脑腹内侧核(ventromedial hypothalamic nucleus, VMH)神经元活动的影响,探讨电针预处理抗MIRI的中枢调控机制。方法 将36只C57BL/6小鼠随机分为假手术组、模型组、电针组,每组12只;电针组选取双侧神门、通里穴进行电针预处理,每次30 min,每日1次,共干预7 d,末次电针预处理结束后1 h内进行MIRI模型复制;模型组和假手术组小鼠不予电针。采用Powerlab多导生理记录仪和Chart软件记录并分析各组小鼠模型复制前、缺血30 min、再灌注120 min时ST段电压值的变化;苏木精—伊红(hemaxylin eosin, HE)染色法观察各组小鼠心肌组织病理变化;免疫荧光染色法观察各组小鼠VMH中c-fos表达情况以及模型组小鼠VMH中c-fos分别与谷氨酸、γ-氨基丁酸共表达情况;采集、分析和比较各组小鼠VMH神经元电活动。结果 与假手术组比较,模型组小鼠结扎30 min及再灌注120 min时ST段电压值均显著升高(P<0.05),HE染色可见小鼠心肌组织损伤严重,VMH中c-fos阳性细胞数显著增加(P<0.05),VMH锥体神经元放电次数显著增加(P<0.05),中间神经元放电次数显著减少(P<0.05),VMH神经元局部场电位频谱能量增强,放电频率较为密集;与模型组比较,电针组小鼠结扎30 min及再灌注120 min时ST段电压值均显著降低(P<0.05),HE染色可见心肌组织损伤较轻,VMH中c-fos阳性细胞数显著减少(P<0.05),VMH锥体神经元放电次数显著减少(P<0.05),中间神经元放电次数差异无统计学意义(P>0.05),VMH神经元局部场电位频谱能量降低,放电频率较为疏散。模型组小鼠VMH中c-fos与谷氨酸能神经元的共表达量高于与γ-氨基丁酸能神经元的共表达量(P<0.05)。结论 电针预处理心经经穴可降低小鼠VMH锥体神经元放电频率和c-fos表达水平,减轻心肌损伤,VMH中谷氨酸能神经元参与了电针抗MIRObjective To investigate the effect of electroacupuncture(EA)pretreatment at heart channel points on the activity of neurons in the ventromedial hypothalamic nucleus(VMH)of mice with myocardial ischemia-reperfusion injury(MIRI),as well as the central regulatory mechanism of EA pretreatment against MIRI.Methods A total of 36 C57BL/6 mice were randomly divided into sham-operation group,model group,and EA group,with 12 mice in each group.The mice in the EA group were given EA pretreatment at bilateral Shenmen and Tongli points for 30 minutes each time,once a day for 7 days in total,and a model of MIRI was established within 1 hour after the last time of EA pretreatment;the mice in the model group and the sham-operation group were not given EA.The Powerlab multi-channel physiological recorder and Chart software were used to record and analyze the change in the voltage of ST segment before modeling,at 30 minutes of ischemia,and at 120 minutes of reperfusion;HE staining was used to observe the histopathological changes of the myocardium;immunofluorescent staining was used to observe the expression of c-fos in the VMH of mice in each group and the co-expression of c-fos with glutamate(Glu)orγ-aminobutyric acid(GABA)in the VMH of the mice in the model group;the electrical activities of neurons in the VMH were collected,analyzed,and compared.Results Compared with the sham-operation group,the model group had a significant increase in the voltage of ST segment at 30 minutes after ligation and 120 minutes of reperfusion(P<0.05);HE staining showed that the mice in the model group had severe myocardial injury,a significant increase in the number of c-fos-positive cells in the VMH(P<0.05),a significant increase in the discharge frequency of pyramidal neurons in the VMH(P<0.05),a significant reduction in the discharge frequency of interneurons(P<0.05),and enhanced spectral energy of local field potential and a relatively intensive discharge frequency of neurons in the VMH.Compared with the model group,the EA group had a signifi
