摘  要:目的探讨APN基因-11426A/G位点多态性及血清APN水平与妊娠期糖尿病(GDM)发生风险及分娩结局的关系。方法以2019年6月至2021年6月在保定市第二医院进行诊断并进行治疗的80例GDM患者(GDM组)作为研究对象,同时选择同期进行检查的妊娠期糖耐量异常者(GIGT组)以及妊娠期糖耐量正常者(NGT组)各80例作为对照组,比较三组患者的APN基因-11426A/G位点多态性及血清APN水平之间的差异,研究APN基因-11426A/G位点多态性及血清APN水平与GDM发生风险及分娩结局的相关性。结果GDM组GG 15.79%、AG 18.89%、AA 58.06%、G 17.16%、A 45.29%,GIGT组为28.07%、36.67%、33.33%、31.86%、34.42%,NGT组为56.14%、44.44%、8.60%、50.98%、20.29%,GDM组、GIGT组、NGT组的-11426A/G的基因型以及等位基因、血清APN之间的差异有统计学意义(P<0.05);通过对两组患者的最小等位基因频率进行对比观察,进入本研究的所有基因均符合温伯格平衡,通过对两组患者的最小等位基因进行对比观察,-11426A/G基因的AA、AG等位基因差异有统计学意义(P<0.05);不良妊娠结局GG 11.11%、AG 58.82%、AA 18.52%、G 34.29%、A 24.00%,正常妊娠结局为88.89%、41.18%、81.48%、65.71%、76.00%,正常妊娠结局与不良妊娠结局的-11426A/G的基因型以及等位基因、血清APN之间的差异有统计学意义(P<0.05);GDM风险与不同妊娠结局与-11426A/G多态性呈现显著的相关性。结论APN基因-11426A/G位点多态性及血清APN水平与GDM发生风险及分娩结局呈现显著的相关性。Objective To investigate the associations between APN gene-1426a/G polymorphism and serum APN level with the risk of GDM and delivery outcome.Methods In this study,a prospective study was conducted in 80 patients with gestational diabetes mellitus(GDM)who were diagnosed and treated in our hospital from June 2019 to June-2021.At the same time,80 cases of gestational impaired glucose tolerance(GIGT)and normal glucose tolerance normal group(NGT)in our hospital during the same period were randomly selected as the control group.To compare the differences of APN gene-11426a/G polymorphism and serum APN level among the three groups,and to study the correlation between APN gene-11426a/G polymorphism and serum APN level and the risk of GDM and delivery outcome.Results The prevalence of GG,AG,AA,G and A were 15.79%,18.89%,58.06%,17.16%and 45.29%,respectively,in GDM group;and 28.07%,36.67%,33.33%,31.86%and 34.42%,respectively,in GIGT group.And they were 56.14%,44.44%,8.60%,50.98%and 20.29%,respectively,in NGT group.There were significant differences in–11426A/G genotype,allele and serum APN among GDM group,GIGT group and NGT group(P<0.05).The minimum allele frequencies for all genes in the study were consistent with the Hardy-Weinberg equilibrium.The differences in AA and AG alleles of-11426A/G gene between the two groups were statistically significant(P<0.05).The differences in-11426A/G genotypes,alleles and serum APN levels between normal and adverse pregnancy outcomes were statistically significant(P<0.05).There was a significant correlation between the risk of GDM and different pregnancy outcomes and-11426A/G polymorphism.Conclusion There is a significant correlation between APN gene-11426A/G polymorphism,serum APN level,and the risk of GDM and delivery outcomes.
