压力性损伤风险无线报警系统在骨折患者术前支具固定中的应用效果研究

摘  要:目的分析压力性损伤风险无线报警系统在骨折支具固定患者中的应用效果。方法收集2020年9月—2022年8月南京医科大学附属南京医院骨科收治的行骨折内固定手术前需支具固定的50例患者资料,根据是否采用压力性损伤风险无线报警系统分为对照组25例和试验组25例。对照组实施常规护理手段,人为进行压力性损伤监测,男性11例,女性14例;年龄34~46岁,平均41.7岁;道路交通伤12例,高处坠落伤4例,摔伤5例,重物砸伤4例。试验组运用压力性损伤风险无线报警系统,设置报警压力为200mmHg,持续受压时间超过60min即发出报警。男性12例,女性13例;年龄36~50岁,平均41.6岁;道路交通伤13例,高处坠落伤3例,摔伤5例,重物砸伤4例。记录两组患者压力性损伤发生情况;运用围术期舒适度量表评估两组患者前后体位舒适度;统计两组患者拆卸支具观察次数;比较两组患者对医务人员工作满意度。结果试验组压力性损伤发生率(压疮发生率=压疮发生例数/总例数×100%)低于对照组(8.0%vs.40.0%,P<0.05);两组患者体位舒适度评分均高于干预前,且试验组体位舒适度评分高于对照组,差异均有统计学意义(P<0.05);试验组患者平均每天拆卸支具次数(3.4±1.4)次少于对照组(7.9±1.7)次(P<0.05)。试验组患者满意度(8.8±1.0)分高于对照组患者满意度(6.9±1.7)分(P<0.05)。结论压力性损伤风险无线报警系统可降低骨折支具固定患者压力性损伤发生率,并提高其体位舒适度,减少医务人员工作量,具有推广价值。Objective To analyze the effect of a wireless alarm system in predicting the risk of developing pressure injuries following brace-wearing in fracture patients before internal fixation.Methods From Sep.2020 to Aug.2022,the data of 50 patients with fractures who were admitted to the Department of Orthopaedics,Nanjing First Hospital and needed brace-wearing before internal fixation were retrospectively analyzed.Patients were divided into control group and experimental group before and after the application of the new alarm system(n=25 for each).In control group,routine nursing and manual monitoring of pressure injuries was conducted,with 11 males and 14 females aged 34-46 years,mean 41.7 years.The causes of injuries were 12 road traffic accidents,4 falls from height,5 ground-level falls and 4 crash by heavy objects.In the experimental group,a wireless alarm system was used for pressure injury monitoring.The warning indicator was set as 200 mmHg pressure continuously for more than 60 min.In this group,there were 12 males and 13 females aged 36-50 years,mean 41.6 years.The causes of injuries were 13 road traffic accidents,3 falls from height,5 ground-level falls and 4 crash by heavy objects.The incidence of pressure injuries following brace-wearing was recorded.The perioperative comfort scale was used to evaluate the comfort level of patients before and after brace fixation.The frequency of removing-brace exams per day was counted,and the satisfaction of medical service was compared between the two groups.Results The incidence of pressure injuries in the experimental group was lower than that in the control group(8.0%vs.40.0%,P<0.05).The perioperative comfort scale of both groups was highly improved than that before brace fixation,and the score was much higher in the experimental group than in the control group(P<0.05).The average number of brace removal exams per day was 3.4±1.4 in the experimental group,lower than that in the control group(7.9±1.7,P<0.05).The satisfaction of medical service was 8.8±1.0 in the exp
