滋阴除痹汤联合温针灸命门穴治疗老年痛风性关节炎的临床研究

摘　　要：目的 探究滋阴除痹汤联合温针灸命门穴治疗老年痛风性关节炎的临床效果。方法 选取2020年5月—2021年12月洛阳市中医院纳入的118例老年痛风性关节炎患者,按随机数字表法分为对照组59例,试验组59例。对照组服用滋阴除痹汤治疗,试验组在此基础上,加用温针灸命门穴,治疗2周。比较两组患者临床治疗效果、治疗前后中医证候积分、超敏C反应蛋白(hs-CRP)、血尿酸(UA)、视觉模拟评分法(VAS)评分,统计并比较两组随访3个月结束时生活质量综合评定问卷(GQOLI-74)评分,记录治疗期间两组患者出现的不良反应。结果 治疗后,试验组临床治疗有效率高于对照组(P<0.05);两组中医证候积分、hs-CRP、UA水平、VAS评分均低于治疗前(P<0.05),试验组低于对照组(P<0.05);随访3个月,两组GQOLI-74评分较治疗前提高(P<0.05),试验组显著高于对照组(P<0.05);比较两组患者不良反应发生情况,差异无统计学意义(P>0.05)。结论 滋阴除痹汤联合温针灸命门穴治疗老年痛风性关节炎的临床效果显著,且安全性良好,能够减轻老年痛风性关节炎患者痛感,降低其血尿酸浓度,减轻炎症反应,帮助患者提高生活质量。Objective To explore the clinical effect of Ziyin Chubi Decoction(滋阴除痹汤)combined with warm acupuncture at Mingmen acupoint on gouty arthritis in the elderly.Methods 118 elderly patients with gouty arthritis who were enrolled in Luoyang Traditional Chinese Medicine Hospital from May 2020 to December 2021 were selected as the research objects.They were randomly divided into control group and combined group,59 cases each.The control group was treated with Ziyin Chubi Decoction,and on this basis,the combined group was treated with warm acupuncture at Mingmen for 2 weeks.The clinical treatment effect,traditional Chinese medicine syndrome score,high sensitivity C reactive protein(hs-CRP),blood uric acid(UA),and visual analog score(VAS)were compared between the two groups before and after treatment.The scores of the comprehensive quality of life assessment questionnaire(GQOLI-74)at the end of the three months of follow-up were counted and compared between the two groups,and the side effects of the two groups during treatment were recorded.Result After treatment,effective rate of clinical treatment in combined group higher than control group(P<0.05).the traditional Chinese medicine syndrome score,hs-CRP,UA level and VAS score of the two groups were lower(P<0.05),and the combined group was lower than the control group(P<0.05).At the end of 3 months of follow-up,the GQOLI-74 scores of the two groups were higher(P<0.05),and the combined group was significantly higher than the control group(P<0.05).There was no statistical significant difference in the occurrence of side effects between the two groups(P>0.05).Conclusion The clinical effect of Ziyin Chubi decoction combined with warm acupuncture at Mingmen acupoint on gouty arthritis in the elderly is significant,and the safety is good.It can reduce the pain of elderly patients with gouty arthritis,reduce the concentration of serum uric acid,reduce the inflammatory reaction,and help patients improve their quality of life.
