中药方剂治疗重症急性胰腺炎及并发症研究进展

摘　　要：重症急性胰腺炎(severe acute pancreatitis,SAP)是急性胰腺炎中的一种特殊类型,可并发心、肺、肝、肾等多器官损伤引起机体多器官功能障碍。中药方剂疗效显著、作用全面,通过组方配伍,能发挥多环节、多途径、多靶点等机制减轻SAP,被广泛应用于SAP的临床治疗。SAP病机特点主要是“热瘀毒虚”,故具有“泻下通腑、清热解毒、活血化瘀、益气扶正”作用的中药方剂对于治疗SAP尤为重要。上述四类方剂能通过改善胰腺缺血、促进胃肠功能恢复和抑制炎症反应等作用改善SAP患者的胰腺、肠、心、肺、肝、肾损伤,并且降低腹腔感染或者全身炎症反应综合征的发生率。该文通过系统阐述“泻下通腑类方、清热解毒类方、活血化瘀类方、补益类方”治疗SAP及其并发症的分子机制及临床应用研究进展,以期为临床用药提供科学依据。Severe acute pancreatitis(SAP)is a particular type of acute pancreatitis that characterized multiorgan dysfunction caused by damage to the heart,lungs,liver,and kidneys.TCM prescriptions have significant curative effect and comprehensive effects.Through the combination of prescriptions,they can play a multi-link,multi-channel and multi-target mechanism to alleviate SAP,and are widely used in the clinical treatment of SAP.The pathogenesis of SAP is mainly characterized by“heat stasis and poison deficiency”,so it is particularly important for the treatment of SAP to have the functions of“purging the Fuorgans,clearing heat and detoxifying,promoting blood circulation and removing blood stasis,invigorating Qi and strengthening resistance”.The above four types of prescriptions can improve pancreatic,intestinal,heart,lung,liver and kidney injuries in SAP patients by improving pancreatic ischemia,promoting gastrointestinal function recovery and inhibiting inflammatory response,and reduce the incidence of abdominal infection or systemic inflammatory response syndrome.In this paper,the molecular mechanism and clinical application of“purging Tongfu prescriptions,clearing heat and detoxifying prescriptions,promoting blood circulation and removing blood stasis prescriptions,tonifying and supplementing prescriptions”in the treatment of SAP and its complications were systematically expounded,in order to provide scientific basis for clinical drug use.
