双心疾病发病的相关机制探讨

摘　　要：时代发展迅速不但给人类带来生活品质的提高,并且给人类带来许多未知的压力,这使得人类疾病模式由生物医学模式逐渐转变为生物-社会-心理医学模式。在前期的研究中,我们发现中医情志刺激与西医慢性应激是导致双心疾病的主要原因,其机制分别为情志刺激影响中医心-脑-肾、慢性应激影响海马和HPA轴,其致病特点非常相似。慢性应激通过影响海马组织中BDNF表达的含量以及刺激外周自主神经的活动等途径,继而改变海马原有的功能,最终形成“双心疾病”,而对于HPA轴来说则是影响了其激素调节的级联机制。中医的情志刺激则是作用于心-脑-肾系统,使得三脏病理相损,终患心疾。深入探究海马和HPA轴与中医心-脑-肾的关系,了解其致病机制,将为中西医治疗“双心疾病”奠定一定的基础。The rapid development of the times not only brings the improvement of human quality of life,but also brings a lot of unknown pressure to human,which makes the human disease model gradually changed from the biomedical model to the biological-social-psychological model. In previous studies,we found that emotional stimulation of traditional Chinese medicine and chronic stress of Western medicine are the main causes of double heart disease. The mechanism is that emotional stimulation affects the heart,brain and kidney of traditional Chinese medicine,and chronic stress affects the hippocampus and HPA axis respectively,and their pathogenic characteristics are very similar. Chronic stress changes the original function of hippocampus through influencing the expression content of BDNF in hippocampus and stimulating the activity of peripheral autonomic nerve,and finally forms“bicardiac diseases”. For HPA axis,it affects the cascade mechanism of its hormone regulation. The emotional stimulation of traditional Chinese medicine acts on the heartbrain-kidney system,resulting in pathological damage of the three viscera and eventual heart disease.An in-depth study of the relationship between hippocampus and HPA axis and heart-brain-kidney in traditional Chinese medicine and its pathogenesis will lay a certain foundation for TCM and Western medicine treatment of“bicardiac diseases”.
