针药结合治疗肾虚肝郁型围绝经期女性失眠临床研究

摘　　要：目的研究针药结合治疗肾虚肝郁型围绝经期女性失眠的临床效果。方法将60例围绝经期肾虚肝郁型失眠患者随机分为研究组和对照组,每组30例。对照组采用口服滋水清肝饮治疗,研究组在对照组基础上给予调任通督针刺法治疗,两组均治疗28 d。观察并比较两组临床疗效,匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI),焦虑自评量表(Self-Rating Anxiety Scale,SAS)评分,雌激素(Estradiol,E_(2))、促卵泡激素(Follicle stimulating hormone,FSH)、促黄体生成素(Luteotropic hormone,LH)水平,更年期女性改良Kupperman评分及更年期生存质量量表(Menopause-specific quality of life questionnaire,MENQOL)评分。结果研究组总有效率显著高于对照组(P<0.05);治疗后两组PSQI各项评分显著降低,研究组显著低于对照组(P<0.05);治疗后两组SAS评分显著降低,研究组显著低于对照组(P<0.05);治疗后两组E_(2)水平显著升高、FSH及LH水平显著降低,并且研究组E_(2)水平显著高于对照组、FSH及LH水平显著低于对照组(P<0.05);治疗后两组Kupperman、MENQOL评分均显著降低,研究组Kupperman、MENQOL评分显著低于对照组(P<0.05)。结论采用滋水清肝饮联合调任通督针刺法治疗围绝经期肾虚肝郁型失眠患者可显著提高临床疗效,改善患者失眠、焦虑症状以及性激素水平,减轻围绝经期临床症状,提高患者的围绝经期生存质量,值得推广应用。Objective To study the clinical effect of acupuncture combined with medicine for treatment of insomnia in perimenopausal women with kidney deficiency and liver stagnation type.Methods Sixty cases of perimenopausal women with kidney deficiency and liver stagnation type were randomly divided into two groups,30 cases in each group.The control group was treated with Zishui Qinggan Decoction(滋水清肝饮)orally,and the research group was given regulating Ren meridian and dredging Du meridian acupuncture on the basis of the control group.Both groups were treated for 28 days.The clinical efficacy,Pittsburgh Sleep Quality Index(PSQI),Self-rating Anxiety Scale(SAS)scores,estrogen(E_(2)),follicle stimulating hormone(FSH),luteinizing hormone(LH),modified Kupperman score for menopausal women and menopause-specific quality of life questionnaire(MENQOL)score of two groups were observed and compared.Results The total effective rate in the research group was statistically higher that of the control group(P<0.05).The scores of PSQI and SAS of the two groups after treatment were significantly reduced and the scores of the research group were significantly lower than those of the control group(P<0.05).After treatment,the E_(2) level of two groups were significantly increased,the levels of FSH and LH were significantly reduced and the E_(2) level of the research group was significantly higher than that of the control group.The levels of FSH and LH of the research group were significantly lower than those of the control group(P<0.05).After treatment,the Kupperman and MENQOL scores of two groups were significantly reduced and the scores of the research group were significantly lower than those of the control group(P<0.05).Conclusion For insomnia in perimenopausal women with kidney deficiency and liver stagnation type,Zishui Qinggan Decoction combined with regulating Ren meridian and dredging Du meridian acupuncturecan significantly improve the clinical efficacy,relieve insomnia and anxiety symptoms and sex hormone levels,reduce the
