摘  要:目的比较颈椎前路零切迹椎间融合器(ROI-C)与传统钛板联合融合器治疗脊髓型颈椎病(CSM)的效果。方法抽取2021年2月至2022年2月郑州人民医院收治的CSM患者76例,按照随机数字表法分为ROI-C组和传统组,每组38例。传统组采用传统钛板联合融合器治疗,ROI-C组采用ROI-C治疗。比较两组围术期指标、脊髓功能和颈椎功能[日本骨科学协会(JOA)评分、颈椎功能障碍指数(NDI)]、椎间隙高度和颈椎生理曲度及并发症发生率。结果ROI-C组术中出血量少于传统组,手术时间、住院时间短于传统组(P<0.05)。术后3个月、6个月、1年,ROI-C组JOA评分高于传统组,NDI评分低于传统组(P<0.05)。两组术前及术后3个月、6个月、1年的椎间隙高度、Cobb角比较差异未见统计学意义(P>0.05);术后3个月、6个月、1年,两组椎间隙高度、Cobb角均较术前增加(P均<0.05)。ROI-C组并发症发生率(2.63%,1/38)低于传统组(21.05%,8/38),P<0.05。结论ROI-C与传统钛板联合融合器应用于CSM中均能改善椎间隙高度和颈椎生理曲度,但与传统钛板联合融合器相比,ROI-C能减少术中出血量,缩短手术时间和住院时间,改善患者脊髓功能和颈椎功能,减少并发症的发生。ObjectiveTo compare the effect of zero-profile anterior cervical cage(ROI-C)and traditional titanium plate combined with cervical cage in the treatment of cervical spondylotic myelopathy(CSM).MethodsA total of 76 patients with CSM treated in People’s Hospital of Zhengzhou from February 2021 to February 2022 were selected,and they were divided into ROI-C group and traditional group according to random number table method,with 38 cases in each group.The traditional group was treated by traditional titanium plate combined with cervical cage,and the ROI-C group was treated by ROI-C.The perioperative indexes,spinal cord function and cervical spine function assessed by Japanese Orthopaedic Association scores(JOA)and neck disability index(NDI),intervertebral height,cervical physiological curvature and incidence of complication were compared between the two groups.ResultsThe intraoperative bleeding volume in the ROI-C group was lower than that in the traditional group,while the surgical time and hospitalization time were shorter than those in the traditional group(P<0.05).The JOA score of the ROI-C group was higher than that of the traditional group,while the NDI score was lower than that of the traditional group 3 months,6 months and 1 year after surgery(all P<0.05).There was no significant difference in vertebral space height and Cobb angle between the two groups before surgery,3 months after surgery,6 months after surgery and 1 year after surgery(P>0.05).The vertebral space height and Cobb angle in both groups increased 3 months,6 months and 1 year after surgery,compared with the two indexes before surgery(all P<0.05).The incidence of complication of the ROI-C group(2.63%,1/38)was lower than that of the traditional group(21.05%,8/38),P<0.05.ConclusionsBoth ROI-C and traditional titanium plate combined with cervical cage can improve the intervertebral height and cervical physiological curvature of CSM patients.However,compared with traditional titanium plate combined with cervical cage,ROI-C can reduce intraoperative
