摘 要:目的利用功能磁共振成像(functional magnetic resonance imaging,f MRI)技术及头针专用头颅线圈(Acupuncture Head Coil 12,AHC12)阐述真假电针刺激百会(GV20)、神庭(GV24)所引起的特异性脑功能变化。方法招募无认知障碍受试者70名,采用3.0T磁共振扫描仪、ACH12,获取电针刺激GV20与GV24针灸前后静息态f MRI图像及电针刺激同时的任务态f MRI图像,比较真假电针刺激头穴的脑功能差异变化。结果真假针刺2组受试者针感量表评分中,酸感、痛感、沉感具有统计学意义;假针刺激组在干预前后脑功能变化差异无统计学意义。真针刺组表现为电针刺激后两侧楔前叶、左侧楔叶、右侧距状裂周围皮层脑区ALFF值增高、左侧岛盖部额下回、左侧额下回三角部Re Ho值增高;真针刺组在任务态时,左侧额中回脑区正激活。结论AHC12及特制磁共振兼容电针刺激仪实现头部穴位电针刺激下的即刻f MRI成像,为穴位特性研究提供了可视化的影像方法,真针刺激神庭与百会后负责认知、记忆、情绪的脑区活动增加。Objective To explore the different brain effects by verum and sham electroacupuncture stimulating baihui(GV20)and shenting(GV24)by functional magnetic resonance imaging(fMRI)and special Acupuncture Head Coil(AHC12).Methods Seventy volunteers without cognitive impairment were recruited.All fMRI examinations were performed on uMR7803.0T MRI scanner with AHC12.Rest fMRI was performed before and after acupuncture.Task fMRI was performed during acupuncture.The changes of brain activity between verum and sham acupuncture at scalp points were compared to understand the value of verum acupuncture.Results There were significant differences between the two groups in sour,painful and heavy feelings.There was no difference in the sham stimulation group before and after acupuncture.In the verum acupuncture group,Amplitude of Low Frequency Fluctuations values of the bilateral precuneus,left cuneus and right talus fissure were increased after electrical stimulation,and Regional Homogeneity values of the left inferior frontal gyrus and triangular inferior frontal gyrus were increased.In the verum acupuncture group,the left middle frontal gyrus was activated during electroacupuncture stumulation.Conclusion AHC12 and special magnetic resonance compatible electroacupuncture stimulator realize fMRI during electroacupuncture stimulation and provide a new method to visualize the effects of electroacupuncture stimulating scalp acupoints.After verum electroacupuncture stimulate shenting and baihui acupoints,the activities of brain areas responsible for cognition,memory and emotion are increased.
