摘　　要：目的 探讨采取闭合撬拨复位经皮螺钉内固定术与斜置塑形钢板内固定术治疗舌形跟骨骨折的疗效对比。方法 回顾性选取四川省骨科医院2016年1月至2018年5月收治的舌形跟骨骨折患者68例,按手术方法的不同将患者分为两组,其中采用闭合撬拨复位经皮螺钉内固定术治疗的35例患者(闭合复位组),余下33例采用斜置塑形钢板内固定治疗(斜置钢板组),观察两组的手术时间、住院时间、骨折愈合时间及术后跟骨高度、B?hler角、并发症和Maryland足踝功能评分。结果 68例患者获得术后3年随访,闭合复位组患者的手术时间、住院时间、骨折愈合时间明显短于斜置钢板组(P<0.05);术后两组患者跟骨高度、B?hler角比较,差异无统计学意义(P>0.05);两组患者并发症总发生率及Maryland足踝功能评分比较,差异无统计学意义(P>0.05)。结论闭合撬拨复位经皮螺钉内固定治疗舌形跟骨骨折能缩短手术时间、住院时间和骨折愈合时间,与斜置塑形钢板内固定相比,在纠正跟骨的高度、B?hler角和并发症方面差异不大。Objective To compare the efficacy of closed prying reduction and percutaneous screw internal fixation with oblique plastic plate internal fixation in the treatment of tongue shaped calcaneal fractures.Methods A total of 68 patients with tongue shaped calcaneal fracture admitted to Sichuan Orthopedic Hospital from January 2016 to May 2018 were retrospectively selected,and the patients were divided into two groups according to the different surgical methods.Among them,35 patients were treated with closed prying reduction and percutaneous screw internal fixation(closed reduction group),and the remaining 33 patients were treated with inclined contoured internal fixation(oblique plate group).The operation time,hospitalization time,fracture healing time and postoperative calcaneal height,B?hler angle,complications and long-term prognosis were observed in both groups.Results A total of 68 cases were followed up for three years after operation.The operation time,hospitalization time and fracture healing time of the closed reduction group were significantly shorter than those of the oblique plate group(P<0.05).There was no significant difference in calcaneus height and B?hler angle between the two groups(P>0.05).There was no significant difference in the total incidence of complications and long-term prognosis between the two groups(P>0.05).Conclusion Closed prying reduction and percutaneous screw internal fixation can shorten the operation time,hospitalization time and fracture healing time of tongue shaped calcaneal fracture.Compared with oblique contoured internal fixation,there is no significant difference in correcting calcaneal height,B?hler angle,short-term and long-term complications.
