摘　　要：目的:探讨动态监测视黄醇结合蛋白4(RBP4)、可溶性CD26(sCD26)、超氧化物歧化酶(SOD)对预测多耐药肺结核(MDR-PTB)疗效的临床价值。方法:选取我院2020年5月-2021年11月收治的147例MDR-PTB患者为研究对象,根据治疗6个月后疗效判断标准分为有效组(102例)和无效组(45例)。比较两组治疗前、治疗2、4、6个月后血清RBP4、sCD26、SOD水平、肺功能指标[第1秒呼气容积占比(FEV1)、最大呼气流速峰值(PEF)、第1秒用力呼气量占用力肺活量比(FEV1/FVC)],分析治疗2、4、6个月后血清RBP4、sCD26、SOD水平与肺功能指标、疗效的相关性。结果:治疗2、4、6个月后,有效组FEV1、PEF、FEV1/FVC、血清RBP4、SOD水平较无效组高,血清sCD26水平较无效组低(P<0.05);治疗2、4、6个月后,血清RBP4、sCD26、SOD水平均与疗效、肺功能紧密相关(P<0.05)。结论:动态监测血清RBP4、sCD26、SOD水平有助于临床早期预测MDR-PTB患者预后。Objective:To explore the clinical value of dynamic monitoring of retinol binding protein 4(RBP4),soluble CD26(sCD26)and superoxide dismutase(SOD)in predicting the efficacy of multidrug resistant pulmonary tuberculosis(MDR-PTB):Methods:147 patients with MDR-PTB admitted to our hospital from May 2020 to November 2021 were selected as the study objects.According to the eficacy after 6 months of treatment,they were divided into the effective group(102 cases)and the ineffective group(45 cases).The levels of serum RBP4,sCD26,SOD and lung function indexes the proportion of expiratory volume in the first second(FEV1),peak expiratory flow rate(PEF),the ratio offorced expiratory volume in the first second to forced vital capacity(FEV1/FVC)were compared between the two groups before treatment and after 2,4 and 6 months of treatment.The correlation between the levels of serum RBP4,sCD26,SOD and lung function indexes and efficacy was analyzed.Results:After 2,4 and 6 months of treatment,the levels of FEV1,PEF,FEV1/FVC,serum RBP4 and SOD in the ineffective group were lower than those in the effective group,and the levels of serum sCD26 were higher than those in the effective group(P<0.05);After 2,4 and 6 months of treatment,serum RBP4,sCD26 and SOD levels were closely related to curative effect and lung function(P<0.05).Conclusion:Dynamic monitoring of serum RBP4,CD26 and SOD levels is helpful to predict the prognosis of MDR-PTB patients in early stage.
