特布他林对臀位外倒转成功的影响因素分析

摘　　要：目的探讨硫酸特布他林对单胎臀位外倒转术(ECV)成功的影响因素。方法收集2018年12月至2021年12月在绍兴市上虞人民医院产科就诊和建孕产期保健册的自愿接受臀位外倒转术(EVC)的孕妇。按操作是否成功分为成功组和失败组,采用单因素分析和多变量Logistic回归进行统计分析。结果共纳入72例孕妇行ECV,47例成功,成功率为65.3%。单因素分析结果显示,羊水指数、臀先露方式、胎盘位置、子宫张力与ECV的结局显著相关,差异有统计学意义(P<0.05)。多变量Logistic回归分析结果显示,子宫张力低、羊水指数≥10 cm、单臀先露、非前壁胎盘是影响ECV成功的保护性因素(OR>1,P<0.05)。成功后头位经阴道分娩率为89.4%(42/47)。结论子宫张力低、非前壁胎盘、水指数≥10 cm、单臀先露是ECV成功的影响因素。臀位ECV可靠性及安全性高,可有效降低剖宫产率和分娩成本,有意愿的孕妇均可评估后实施。Objective To explore the factors influencing the success of terbutaline sulfate in single fetal breech external inversion(ECV).Methods Pregnant women who voluntarily received external cephalic version(EVC)and gave birth in Shangyu People's Hospital of Shaoxing city with complete maternity and perinatal health records during December 2018 to December 2021 in the department of Obstetrics were enrolled in the study.Subjects were divided into the successful group and the failed group.The related factors were analyzed by univariate analysis and multivariate Logistic regression analysis.Results Among the 72 pregnant women who underwent ECV,47 cases(65.3%)were successful.The results of monofactor analysis between the successful group and the failure group showed that factors influencing the success of the operation were amniotic fluid index,the buttock type,the position of placenta and tension of the uterus(all P<0.05).Multivariate analysis showed that the success rate was dependent on the low tension of the uterus,amniotic fluid index≥10 cm,single breech presentation,non-anterior wall placenta(OR>1,P<0.05).In the 47 cases successful EVC cases,89.4%of women(42cases)delivered vaginally.Conclusion The low tension of the uterus,non-anterior wall placenta,amniotic fluid index≥10 cm and single breech presentation are the main influencing factors of the success rate of ECV.EVC has high reliability and safety,which can effectively reduce the cesarean section rate and delivery cost in Shangyu area.Pregnant women who are willing can try to perform it after evaluation.
