经皮穿刺面神经电刺激联合星状神经阻滞治疗恢复期贝尔麻痹的疗效

摘　　要：目的探讨经皮穿刺面神经电刺激联合星状神经阻滞对恢复期贝尔麻痹的疗效。方法选取2019年7月至2022年11月贝尔麻痹患者90例。根据简单随机化法分为观察组与对照组,每组各45例。对照组在超声引导下经皮穿刺达面神经茎乳孔出口处靶点位置,予复方倍他米松与甲钴胺混合液患侧面神经阻滞,并联合同侧星状神经阻滞;观察组在对照组基础上穿刺达靶点位置后连接神经刺激器行电刺激治疗20 min。比较两组患者治疗前后总有效率、HBGS量化转换表的估计功能值、TFGS的综合评分。结果观察组总有效率高于对照组(P<0.05),治疗后患者HBGS量化转换表的估计功能值与TFGS的综合评分均高于治疗前,且观察组高于对照组(P<0.05)。结论经皮穿刺面神经电刺激联合星状神经阻滞对恢复期贝尔麻痹患者的治疗效果显著,明显改善面神经功能,促进患者尽快恢复,减少心理精神障碍,提高生活质量,值得临床进一步探索。Objective To study the therapeutic effects of percutaneous puncture facial nerve electrophotoluminescence combined with stellate nerve block in convalescent Bell's paralysis.Methods Ninety patients with Bell's paralysis enrolled in our hospital from July 2019 to November 2022 were selected and divided into the treatment group and the control group according to simple randomization method,with 45 cases in each group.For the control group,after percutaneous puncture reached the target of exit of facial nerve stylomastoid foramen under the guidance of ultrasonic,it retarded by injecting the mixture of compound betamethasone injection and mecobalamine injection to the affected side of the facial nerve,combining ipsilateral stellate ganglion.As for the treatment group,based on the control group,after percutaneous puncture reached the target,cured it through electrostimulation by connecting the nerve stimulation for 20 minutes.Compare the total effective rate before and after the treatment,the estimated function value of the House-Brackmann grading method(HBGS)quantification conversion table and the comprehensive score of torontofacial gmaing system(TFGS).Results The total effective rate of the treatment group was higher than that of the control group(P<0.05).The estimated function value of the patients'HBGS quantification conversion table and the comprehensive score of TFGS were both higher after the treatment.And the treatment group was also higher than the control group(P<0.05).Conclusion The therapeutic effects of percutaneous puncture facial nerve electrophotoluminescence combined with stellate nerve block is remarkable.It significantly improves facial nerve function,promotes speedy recovery,reduces psychological disorders and improves the quality of life for patients.It deserves further clinical research.
