间苯三酚联合地塞米松在难复性肠套叠空气灌肠中的应用

摘　　要：目的探讨间苯三酚联合地塞米松在难复性肠套叠空气灌肠中应用效果。方法2019年7月至2022年6月因肠套叠在本院急诊初次行肠套叠空气灌肠整复尝试未成功的患儿60例,分为对照组和观察组各30例,对照组在初次肠套叠整复时最大压力维持在12kpa未成功的情况下休息60 min后再次予以空气整复,灌肠压力最大安全值升至13.3 kpa为限。观察组再次整复前1 h予以间苯三酚及地塞米松静脉注射,复位成功后2 d继续予以静脉注射上述药物,1次/d,比较两组患儿第2次空气灌肠整复的成功率,复位后3 d内复套率及观察组的用药不良反应。结果对照组二次空气灌肠整复成功18例,12例失败者3例予以手术,其余9例转往上级医院治疗。成功的18例患儿3 d内复套6例,均再次予以X线下空气灌肠整复成功。观察组二次整复成功24例,6例失败者1例予以手术,其余5例因转往上级医院治疗。成功的24例患儿3 d内复套4例,均再次予以X线下空气灌肠整复成功。结论间苯三酚联合地塞米松在难复性肠套叠二次空气整复时可提高成功率,降低复位后短期内复套率。Objective To investigate the effect of phloroglucinol combined with dexamethasone in irreducible intussusceptive air enema.Methods From July 2019 to June 2022,60 children who underwent the first unsuccessful attempt of intussusception air enema in our emergency department due to intussusception were divided into the control group and the observation group,with 30 cases in each group.In the control group,the maximum pressure during the first unsuccessful intussusception was maintained at 12 kpa and the control group was given air enema again after 60 minutes of rest.The maximum safe value of enema pressure was increased to 13.3 kpa.The observation group was given intravenous injection of phloroglucinol and dexamethasone 1 h before re-rectification,and continued to be given intravenous injection of the above drugs 2 days after successful reduction,once a day.The success rate of the second air enema of the two groups,the rate of re-insertion within 3 days after reduction and the adverse reactions of the observation group were compared.Results In the control group,18 cases were successfully treated with secondary air enema,12 cases were unsuccessful,3 cases were operated,and the other 9 cases were transferred to superior hospital for treatment.Of the 18 children,6 cases were successfully reperformed within 3 days,and all of them were successfully reperformed by air enema under X-ray line.In the observation group,24 cases were successfully reconstructed twice,1 case of 6 losers was operated on,and the other 5 cases were transferred to a higher hospital for treatment.Of the 24 children,4 cases were successfully reperformed within 3 days,and all of them were successfully reperformed by air enema under X-ray.Conclusion Phloroglucinol combined with dexamethasone can improve the success rate and reduce the rate of re-intussusception in the short term.
