主动呼吸循环技术联合振动排痰治疗老年肺部感染患者的效果

摘　　要：目的:观察主动循环呼吸技术(ACBT)联合振动排痰治疗老年肺部感染患者的效果。方法:回顾性分析2021年11月至2023年5月该院收治的121例老年肺部感染患者的临床资料,按照治疗方法不同将其分为对照组54例和观察组67例。两组均进行抗感染治疗,在此基础上,对照组予以振动排痰治疗,观察组在对照组基础上联合ACBT治疗,两组均持续治疗1周。比较两组临床疗效、临床症状(咳嗽咳痰、呼吸困难、肺部啰音)消失时间、治疗前后炎性指标[超敏C反应蛋白(hs-CRP)、中性粒细胞计数(NEUT)、淋巴细胞计数]水平和临床肺部感染(CPIS)评分。结果:观察组治疗总有效率为97.01%(65/67),高于对照组的81.48%(44/54),差异有统计学意义(P<0.05);观察组咳嗽咳痰、呼吸困难、肺部啰音等临床症状消失时间均短于对照组,差异有统计学意义(P<0.05);治疗后,观察组hs-CRP、NEUT、淋巴细胞计数水平和CPIS评分均低于对照组,差异有统计学意义(P<0.05)。结论:ACBT联合振动排痰治疗老年肺部感染患者可提高治疗总有效率,促进临床症状消失,降低炎性指标水平和CPIS评分,效果优于单纯振动排痰治疗。Objective:To observe effects of active cycle of breathing technique(ACBT)combined with vibration expectoration in treatment of elderly patients with pulmonary infection.Methods:The clinical data of 121 elderly patients with pulmonary infection admitted to this hospital from November 2021 to May 2023 were retrospectively analyzed.According to different treatment methods,they were divided into control group(54 cases)and observation group(67 cases).Both groups were treated with anti-infection.On this basis,the control group was treated with vibration expectoration,while the observation group was treated with ACBT on the basis of the control group.Both groups were treated for 1 week.The clinical efficacy,the disappearance time of clinical symptoms(cough and expectoration,dyspnea,pulmonary rales),the levels of inflammatory indexes[high-sensitivity C-reactive protein(hs-CRP),neutrophil count(NEUT),lymphocyte count],and the clinical pulmonary infection score(CPIS)were compared between the two groups before and after the treatment.Results:The total effective rate of the observation group was 97.01%(65/67),which was higher than 81.48%(44/54)of the control group,and the difference was statistically significant(P<0.05).The disappearance time of clinical symptoms such as cough and expectoration,dyspnea and pulmonary rales in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.05).Further,after the treatment,the levels of hs-CRP,NEUT,lymphocyte count and the CPIS score in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:ACBT combined with vibration expectoration in the treatment of the elderly patients with pulmonary infection can improve the total effective rate of treatment,promote the disappearance of clinical symptoms and reduce the levels of inflammatory indexes and the CPIS scores.Moreover,it is superior to single vibration expectoration treatment.
