摘  要:目的:观察血清N末端B型脑钠肽原(NT-proBNP)、D-二聚体(D-D)、胱抑素-C(Cys-C)水平联合检测在慢性心力衰竭(CHF)诊断中的效能。方法:选取2021年3月至2023年3月该院收治的78例CHF患者设为观察组,另选取同期78名健康体检者设为对照组,比较两组及不同心功能分级CHF患者血清NT-proBNP、D-D、Cys-C水平,采用Pearson相关性分析血清NT-proBNP、D-D、Cys-C水平与美国纽约心脏病学会(NYHA)心功能分级的相关性,绘制工作特征曲线(ROC)分析血清NT-proBNP、D-D、Cys-C水平单项及联合检测诊断CHF的效能。结果:观察组NT-proBNP、D-D、Cys-C水平均高于对照组,差异有统计学意义(P<0.05);不同NYHA心功能分级CHF患者血清NT-proBNP、D-D、Cys-C水平比较,Ⅰ级<Ⅱ级<Ⅲ级<Ⅳ级,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,血清NT-proBNP、D-D、Cys-C水平与NYHA心功能分级呈正相关(r>0,P<0.05);ROC曲线分析结果显示,血清NT-proBNP、D-D、Cys-C水平单项及联合检测诊断CHF的曲线下面积分别为0.774、0.810、0.784、0.930,均具有一定诊断价值,且联合诊断效能最高。结论:血清NT-proBNP、D-D、Cys-C水平联合检测诊断CHF的效能均高于三者单项检测。Objective:To observe efficiency of combined detection of serum N-terminal B-type brain natriuretic peptide(NT-proBNP),D-dimer(D-D)and cystatin-C(Cys-C)levels in diagnosis of chronic heart failure(CHF).Methods:78 patients with CHF admitted to this hospital from March 2021 to March 2023 were selected as the observation group,and 78 healthy subjects were selected as the control group.The serum levels of NT-proBNP,D-D and Cys-C were compared between the two groups and the CHF patients with different cardiac function classifications.Pearson correlation analysis was used to analyze the correlations among the serum NT-proBNP,D-D,Cys-C levels and the New York Heart Association(NYHA)cardiac function classification.The receiver operating characteristic(ROC)curve was drawn to analyze the efficiencies of single and combined detection of serum NT-proBNP,D-D and Cys-C levels in the diagnosis of CHF.Results:The levels of NT-proBNP,D-D and Cys-C in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of serum NT-proBNP,D-D and Cys-C in the CHF patients with different NYHA cardiac function classifications were compared and showed that:classification I<classification II<classification III<classification IV,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the serum NT-proBNP,D-D and Cys-C levels were positively correlated with the NYHA cardiac function classification(r>0,P<0.05).The ROC curve analysis showed that the area under the curves of single and combined detection of serum NT-proBNP,D-D and Cys-C levels in the diagnosis of CHF were 0.774,0.810,0.784 and 0.930,separately,all of which had certain diagnostic value,and that of the combined diagnostic efficiency was the highest.Conclusions:The efficiency of combined detection of serum NT-proBNP,D-D and Cys-C levels in the diagnosis of CHF is higher than that of single detection of the three.
