肩关节镜术后镇痛的研究进展

摘　　要：随着关节镜技术的发展和应用,肩关节镜手术在骨科手术中所占的比例也在增加,所以良好的肩关节镜手术术后镇痛是现在一个重要的研究方向,现在对于不同的镇痛技术在肩关节镜术后镇痛效果都有一定的研究,还有一些新的镇痛方式在肩关节镜术后镇痛中的应用也在不断地探索,本文主要汇总了一些在肩关节镜中常见的镇痛方式的应用效果,例如肌间沟臂丛神经阻滞、肩胛上神经阻滞联合腋神经阻滞、竖脊肌神经阻滞、臂丛上干神经阻滞、关节周围浸润等在肩关节镜术后的应用效果(几种镇痛方式的术后镇痛,减少阿片类药品应用,以及术后不良反应的发生)。在以上的几种镇痛方式中,臂丛上干神经阻滞可以成为代替肌间沟臂丛神经阻滞成为肩关节镜手术术后镇痛的选择。With the application and development of arthroscopic technology,the proportion of shoulder arthroscopic surgery in orthopedic surgery is also increasing.Therefore,postoperative analgesia after shoulder arthroscopy is an important research direction.There now lies some research on the analgesic effects of different pain relief techniques in shoulder arthroscopic surgery,furthermore,some new pain relief methods being explored in the application of postoperative pain relief after shoulder arthroscopic surgery.This article mainly summarized the application effects of some common pain relief methods in shoulder arthroscopy,such as intermuscular sulcus brachial plexus block,suprascapular nerve block combined with axillary nerve block,erector spinalis nerve block,brachial plexus superior trunk nerve block,and periarticular infiltration targeting on reducing the use of opioids and the occurrence of postoperative adverse reactions.Among the afore-mentioned analgesic methods,brachial plexus superior trunk nerve block can become a choice to replace intramuscular sulcus brachial plexus block as a postoperative analgesic option for shoulder arthroscopic surgery.
