摘　　要：目的:探讨自体富血小板血浆配合手法治疗肩周炎的效果及安全性。方法:选取2020年1月—2021年12月肇庆市第一人民医院的肩周炎患者80例,随机分为观察组与对照组,各40例。对照组采用手法治疗,观察组在对照组基础上采用自体富血小板血浆(PRP)局部注射治疗。治疗后,比较两组Constant-Murley肩关节功能评分、疼痛评分及临床疗效。结果:治疗4周后,观察组疼痛程度较对照组低,差异有统计学意义(P<0.05)。治疗后6个月,两组Constant-Murley肩关节功能各方面评分均高于治疗前,且观察组均高于对照组,差异均有统计学意义(P<0.05);观察组治疗总有效率为87.50%,高于对照组的65.00%,差异有统计学意义(P<0.05)。治疗过程中,观察组不良反应总发生率为10.00%,对照组为7.50%,两组相比差异无统计学意义(P>0.05)。结论:自体PRP配合手法治疗肩周炎,不仅能够有效缓解临床症状,减轻疼痛,还能改善肩关节功能,且有较好的安全性。Objective:To investigate the efficacy and safety of autologous platelet-rich plasma(PRP)combined with manual therapy in the treatment of scapulohumeral periarthritis.Method:From January 2020 to December 2021,80 patients with scapulohumeral periarthritis in the First People's Hospital of Zhaoqing were selected and randomly divided into an observation group and a control group,with 40 cases in each group.The control group was treated with manual treatment,and the observation group was treated with autologous PRP local injection on the basis of the control group.After the treatment,Constant-Murley shoulder function score,pain score and clinical efficacy were compared between the two groups.Result:After 4 weeks of treatment,the degree of pain in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).6 months after treatment,the Constant-Murley shoulder function scores of the two groups were higher than those before treatment,and those of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05);the total effective rate of the observation group was 87.50%,which was higher than 65.00%of the control group,with a statistically significant difference(P<0.05).The process of the treatment,the total incidence of adverse reactions was 10.00%in the observation group and 7.50%in the control group,there was no significant difference between the two groups(P>0.05).Conclusion:Autologous PRP combined with manual therapy in the treatment of scapulohumeral periarthritis can not only effectively relieve the clinical symptoms and pain,but also improve the shoulder joint function of patients,with good safety.
