自拟补肾助阳散敷脐联合骶神经根功能性磁刺激治疗脊髓损伤并发神经源性膀胱的临床研究

摘　　要：目的:探究自拟补肾助阳散敷脐联合骶神经根功能性磁刺激(FMS)治疗脊髓损伤并发神经源性膀胱的临床价值。方法:选取2018年1月—2021年12月新余市中医院收治的64例脊髓损伤并发神经源性膀胱患者,按随机数字表法为对照组(给予常规膀胱功能干预+骶神经根FMS治疗)与研究组(联合自拟补肾助阳散敷脐治疗),各32例。比较两组膀胱功能、临床疗效、中医症状积分及生活质量。结果:治疗后,两组单次排尿量均升高,且研究组高于对照组(P<0.05),两组尿失禁次数、残余尿量、排尿次数均减少,且研究组均少于对照组(P<0.05);研究组总有效率为93.75%,高于对照组的75.00%,差异有统计学意义(P<0.05);治疗后,两组中医症状积分均降低,且研究组均低于对照组(P<0.05);治疗后,两组导尿评分、尿失禁生活质量问卷(I-QOL)评分均升高,且研究组均高于对照组(P<0.05)。结论:自拟补肾助阳散敷脐联合骶神经根FMS治疗脊髓损伤并发神经源性膀胱可改善患者膀胱功能,提高临床疗效,缓解中医症状,提高患者生活质量。Objective:To explore the clinical value of umbilical compress with self-made Bushen Zhuyang Powder combined with sacral nerve root functional magnetic stimulation(FMS)in the treatment of spinal cord injury complicated with neurogenic bladder.Method:A total of 64 patients with spinal cord injury complicated with neurogenic bladder treated in Xinyu Hospital of Traditional Chinese Medicine from January 2018 to December 2021 were selected and divided into the control group(received routine bladder function intervention+sacral nerve root FMS treatment)and the study group(combined with umbilical compress with self-made Bushen Zhuyang Powder)according to random number table method,with 32 cases in each group.Bladder function,clinical efficacy,TCM symptom integral and quality of life were compared between the two groups.Result:After treatment,the single urination volume of both groups were increased,and that in the study group was higher than that in the control group(P<0.05),the urinary incontinence times,residual urine volume and urination times of both groups were reduced,and those in the study group were less than those in the control group(P<0.05).The total effective rate of the study group was 93.75%,which was higher than 75.00%of the control group,the difference was statistically significant(P<0.05).After treatment,the scores of TCM symptoms of both groups were decreased,and those in the study group were lower than those in the control group(P<0.05).After treatment,the scores of urethral catheterization and incontinence quality of life questionnaire(I-QOL)of both groups were increased,and those in the study group were higher than those in the control group(P<0.05).Conclusion:Umbilical compress with self-made Bushen Zhuyang Powder combined with sacral nerve root FMS in the treatment of spinal cord injury complicated with neurogenic bladder can improve bladder function,improve clinical efficacy,relieve symptoms of TCM and improve patients'quality of life.
