腹股沟上髂筋膜神经阻滞联合腘窝坐骨神经阻滞在下肢动脉硬化球囊扩张支架植入术中的应用

摘　　要：目的观察腹股沟上髂筋膜神经阻滞联合腘窝坐骨神经阻滞在下肢动脉硬化球囊扩张支架植入术中的应用。方法将2021年2月—2023年2月三明市第二医院收治的以下肢动脉硬化球囊扩张支架植入术治疗的96例患者作为研究对象,根据麻醉方式不同分为两组,其中48例采用腹股沟上髂筋膜神经阻滞联合腘窝坐骨神经阻滞为研究组,以全身麻醉的另48例为对照组。比较不同时间点的血流动力学指标,术后不同时间段内的疼痛评分及在术后的并发症发生情况。结果研究组在入室10 min后(T_(1))、手术开始后30 min(T_(2))血流动力学指标较对照组更平稳,差异有统计学意义(P<0.05)。研究组患者术后不同时间段疼痛评分低于对照组(P<0.05)。研究组麻醉后并发症总发生率低于对照组(P<0.05)。结论针对在下肢动脉硬化球囊扩张支架植入术,以腹股沟上髂筋膜神经阻滞联合腘窝坐骨神经阻滞麻醉可显著稳定术中生命体征指标,降低术后疼痛,减少术后并发症发生,麻醉安全且效果理想。Objective To observe the application of superior inguinal iliac fascia nerve block combined with popliteal sciatic nerve block in lower limb arteriosclerosis balloon dilation stent implantation.Methods A total of 96 patients admitted to Sanming Second Hospital from February 2021 to February 2023 for the treatment of lower limb arteriosclerosis with balloon dilation stent implantation were divided into two groups based on different anesthesia methods.Among them,48 cases were treated with inguinal iliac fascia nerve block combined with popliteal sciatic nerve block as the study group,while the other 48 cases were treated with general anesthesia as the control group.The hemodynamic indicators at different time points,pain scores at different time periods after surgery,and the incidence of postoperative complications were compared.Results The hemodynamic indexes of the study group were more stable than those of the control group 10 minutes after entry(T_(1))and 30 min after operation(T_(2)),and the difference was statistically significant(P<0.05).The pain scores of the study group were lower than those of the control group(P<0.05).The total incidence of postoperative complications in study group was lower than that in control group(P<0.05).Conclusion For lower limb arteriosclerosis balloon dilation stent implantation,the combination of inguinal superior iliac fascia nerve block and popliteal sciatic nerve block anesthesia can significantly stabilize intraoperative vital signs,reduce postoperative pain,and reduce postoperative complications.The anesthesia is safe and effective.
