软式支气管镜治疗SMPP的疗效及对患儿血清IL-8、sIL-2R水平的影响

摘　　要：目的 探究软式支气管镜治疗重症肺炎支原体肺炎(SMPP)患儿的疗效以及对其血清IL-8(IL-8)、可溶性白细胞介素2受体(SIL-2R)水平的影响。方法 选取2021年1月至2021年6月期间于唐山市妇幼保健院接受软式支气管镜治疗的SMPP患儿164例,设为观察组;另选取同期164例接受常规治疗的SMPP患儿设为对照组。对比两组患儿治疗疗效、恢复进度、外周血IL-8、sIL-2R监测结果及不良事件发生情况。结果 观察组的治疗疗效(92.68%)高于对照组(75.61%),差异有统计学意义(χ^(2)=17.918,P<0.05);观察组患儿退热/咳嗽好转/肺部阴影消失/住院时间均短于对照组,差异有统计学意义(t=-12.508、-23.510、-19.710、-18.767,P<0.05);治疗后第3、7 d,观察组患儿血清IL-8、sIL-2R浓度均低于对照组,差异有统计学意义(t_(IL-8)=-4.803、-8.878,t_(sIL-2R)=-2.792、-2.343,P<0.05);观察组不良事件总发生率(4.27%)与对照组(6.71%)比较,差异无统计学意义(χ^(2)=0.263,P>0.05)。结论 软式支气管镜可提高SMPP患儿疗效,降低血清IL-8、SIL-2R水平,缓解机体炎症,且具有一定的安全性。Objective To investigate the effects of flexible bronchoscope in the treatment children with severe Mycoplasma pneumoniae pneumonia(SMPP)and the influence on serum interleukin-8(IL-8)and soluble interleukin 2 receptor(SIL-2R)levels.Methods A total of 164 children with SMPP who underwent flexible bronchoscopic treatment in Tangshan Maternal and Child Health Hospital from January to June 2021 were selected as the observation group.At the same time,164 children with SMPP who underwent convention-al treatment were selected as the control group.Curative effect,recovery progress,monitoring results of pe-ripheral blood IL-8 and sIL-2R,and the occurrence of adverse events were compared between the two groups.Results The treatment response rate in the observation group(92.68%)was higher than that in the control group(75.61%),with a statistically significant difference(X^(2)=17.918,P<0.05).The fever abatement time,improvement time of cough,disappearance time of lung shadow and length of hospital stay in the observation group were shorter than those in the control group,with statistically significant differences(t=-12.508,-23.510,-19.710,-18.767,P<0.05).Serum IL-8 and sIL-2R levels in the observation group on day 3 and day 7 after treatment were lower than those in the control group,with statistically significant differences(tIL-8=-4.803,-8.878,tsIL-2R=-2.792,-2.343,P<0.05).The total incidence rates of adverse events in the observation group and the control group were 4.27%and 6.71%,without statistically significant difference between the groups(X^(2)=0.263,P>0.05).Conclusion Flexible bronchoscope can improve the curative effect on children with SMPP,reduce serum IL-8 and SIL-2R levels,and relieve inflammation with certain safety.
