摘 要:目的 分析结核性脑膜炎(TBM)患者外周血及脑脊液(CSF)中微小RNA-155(miRNA-155)、微小RNA-125b(miRNA-125b)、可溶性CD163(sCD163)表达的临床意义。方法 选取2018年1月至2023年1月于新乡医学院第一附属医院就诊的183例TBM患者作为TBM组,根据其病情分为Ⅰ期57例,Ⅱ期73例和Ⅲ期53例。另选同期60例原发性头痛患者作为对照组,以逆转录聚合酶链反应(RT-PCR)测定各组外周血及CSF中miRNA-155、miRNA-125b表达,以酶联免疫吸附(ELISA)法测定外周血及CSF中sCD163表达。绘制受试者工作特征(ROC)曲线评估外周血及CSF中miRNA-155、miRNA-125b、sCD163表达对TBM的早期诊断价值。分析外周血及CSF中miRNA-155、miRNA-125b、sCD163表达与TBM病情程度的关系。结果 TBM组患者外周血及CSF中miRNA-155、miRNA-125b、sCD163表达水平均显著高于对照组,差异有统计学意义(P<0.05);外周血中miRNA-155、miRNA-125b、sCD163诊断TBM的ROC曲线下面积(AUC)依次为0.886、0.865、0.782,CSF中miRNA-155、miRNA-125b、sCD163诊断TBM的AUC依次为0.925、0.905、0.915(P均<0.05);TBM患者外周血和CSF中miRNA-155、miRNA-125b、sCD163表达水平比较:Ⅲ期>Ⅱ期>Ⅰ期,差异有统计学意义(P<0.05);Spearman相关性分析显示,TBM患者外周血及CSF中miRNA-155、miRNA-125b、sCD163表达水平与其病情程度分别呈正相关关系(r=0.810、0.562、0.325、0.611、0.472、0.682,P<0.05)。结论 TBM患者外周血及CSF中miRNA-155、miRNA-125b、sCD163均显著升高,此三项指标可作为TBM患者临床诊断和病情评估的生物学标志物。Objective To analyze the clinical significance of the expression of microRNA-155(miRNA-155),microRNA-125b(miRNA-125b)and soluble CD163(sCD163)in peripheral blood and cerebrospinal fluid(CSF)of patients with tuberculous meningitis(TBM).Methods A total of 183 patients with TBM who were admitted to the First Affiliated Hospital of Xinxiang Medical University from January 2018 to January 2023 were selected,including 57 patients in stageⅠ,73 patients in stageⅡand 53 patients in stageⅢ.At the same time,60 patients with primary headache were selected as the control group.The expression of miRNA-155 and miRNA-125b in peripheral blood and CSF of each group was measured by reverse transcription polymerase chain reaction(RT-PCR).The expression of sCD163 in peripheral blood and CSF was measured by enzyme-linked immunosorbent assay(ELISA).The diagnostic value of miRNA-155,miRNA-125b and sCD163 in peripheral blood and CSF for TBM in early stage was analyzed using the receiver operating characteristic(ROC)curve.The relationship between the expression of miRNA-155,miRNA-125b and sCD163 in peripheral blood and CSF and the severity of TBM was discussed.Results The expression levels of miRNA-155,miRNA-125b and sCD163 in peripheral blood and CSF in the TBM group were significantly higher than those in the control group(P<0.05).The area under the curve(AUC)values of miRNA-155,miRNA-125b and sCD163 in peripheral blood for diagnosing TBM were 0.886,0.865 and 0.782.The AUC values of miRNA-155,miRNA-125b and sCD163 in CSF for diagnosing TBM were 0.925,0.905 and 0.915(P<0.05).The expression of miRNA-155,miRNA-125b and sCD163 in peripheral blood and CSF of patients with TBM increased in order from stageⅢ,stageⅡto stage I(P<0.05).Spearman correlation analysis found that the expression levels of miRNA-155,miRNA-125b and sCD163 in peripheral blood and CSF of patients with TBM were positively correlated with the severity(r=0.810,0.562,0.325,0.611,0.472,0.682,P<0.05).Conclusion The levels of miRNA-155,miRNA-125b and sCD163 in peripher
